摘 要:目的:观察佳乐施急性扩容对休克患者全麻诱导期血流动力学的影响。方法:选择休克患者40例,随机分为两组,每组20例。A组输入复方氯化钠,B组输入佳乐施,输液速度都为20mL.kg-1.h-1。观察记录气管插管前5min、插管后即刻,插管后5、10、15min收缩压(SBP)、舒张压(DBP)、中心静脉压(CVP)和心率(HR)。结果:两组患者全麻诱导前后各时段收缩压、舒张压及心率相比较无明显差异(P>0.05),佳乐施组插管后15min中心静脉压明显高于复方氯化钠组(P<0.01)。结论:急性扩容不论是晶体或胶体液均可减轻全麻诱导对血流动力学的影响,佳乐施升高中心静脉压优于复方氯化钠。Objective: To observe the influences of rapid volume expansion of gelofusine on hemodynamics during reduction of general anesthesia for the shock patients. Methods: 40 cases of shock patients were randomly divided into two groups with 20 cases each. Group A was infused with compound sodium chloride and injection, and Group B with gelofusine. The transfusion speed in both groups was 20mL ·kg^-1·h^-1. SBP,DBP, CVP and HR were monitored and recorded 5rain before intubation, immediately and 5, 10, 15min after intubation. Results: There were no significant differences between the two groups before and after the induction (P〉0. 05). At 15min after intubation,CVP in group B was significantly higher than that in group A. Conclusion: Infusion of crystalloid solutions or colloid solutions for rapid volume expansion can lessen the hemodynamic influences during induction of general anesthesia. Gelofusion is more effective than compound sodium chloride injection for increasing CVP.
