纤维支气管镜对大咯血患者手术止血定位的价值

摘　　要：目的评价纤维支气管镜对大咯血患者手术止血定位的价值。方法11例因大咯血而内科治疗无效的患者,6例患者利用纤维支气管镜经气管插管或开胸后气管侧切口入支气管,判断出血部位后行肺段或肺叶切除术止血;5例患者通过肺部多层螺旋CT和临床体征判断出血部位,再行肺段或肺叶切除术止血。结果经纤维支气管镜定位后的6例大咯血患者,术后未出现大咯血;经肺部多层螺旋CT和临床体征定位的5例大咯血患者,有2例因再次大咯血二次手术,1例因再次大咯血死亡。结论纤维支气管镜可以准确判定大咯血所在肺段或肺叶的位置,对确定手术治疗方案有很大辅助作用。[Objectivel To evaluate the value of fibrobronchoscope in locating for surgical hemostasis of massive hemoptysic patients. [Methods] Eleven patients who had been suffered from massive hemoptysis were collected. In order to judge the hemorrhagic sites, 6 patients were given tracheal intubation via fibrobronchoscope or thoracotomy and lateral incision of trachea to enter the trachea; while the rest （5 patients） were based on lung CT and clinical sign. Then all of them were given segmentectomy or lobectomy. [Results] The 6 patients who were locating via fibrobronchoscope did not relapse, while among the 5 patients locating via lung CT and clinical signs, 1 patient died from another massive hemoptysis and 2 patients were given second operation because of second hemoptysis. [ Conclusions] Fibrobronchoscope can confirm the location of the massive hemoptysis accurately. It plays an important auxiliary role in ascertaining the scheme of surgical therapy.
