摘 要:目的探讨尿毒症所致肺损害的胸部X线表现及预后。方法对24例尿毒症肺损害患者(尿毒症肺组)进行分析,并与16例非尿毒症肺害患者(非尿毒症肺组)相比较。结果尿毒症所致肺损害的胸部X线表现多种多样,血液透析后可得到改善;与非尿毒症肺组相比尿毒症组的血红蛋白(H b)、血浆蛋白(ALB)、二氧化碳结合力(CO2CP)降低(均P<0.05),而血尿素氮(BUN)、血肌酐(SCr)升高(均P<0.05),尿毒症肺的发生与透析不充分和透析间期体重增长过多有关。结论尿毒症所致的肺损害在临床上较常见,应予以重视;其发病机制复杂,胸部X线检查对诊断有一定帮助,血液透析是一种有效的治疗措施。Objective To investigate the manifestation of X-ray chest films and prognosis of pulmonary damage caused by uremia. Methods Twenty-four cases of uremic patients with pulmonary damage (uremic lung group)were analyzed and compared with 16 cases of uremic patients without pulmonary damage(mon-uremic lung group): Results The manifestations of clinic and X-ray chest films in 24 cases of uremic patients with pulmonary damage were various and improved after hemedialysis. Hemoglobin (Hb),albumin (ALB), carbon dioxide combining power (CO2CP) in uremic lung group were lower(p〈0.05)and blood urea nitrogen(BUN),serum creatinine (SCr)were higher(p〈0.05) than those of non-uremic lung group.The incidence of uremic lung was associated with inadequate dialysis and more hemedialytic weight gains. Conclusions Pulmonary damage caused by uremia was common in the clinic,which should be stressed, hs pathogenesis was complicated ,X-ray chest examination was to some extent helpful to diagnosis,hemedialvsis was an efficient measure in the treatment.
