摘  要:目的建立检测人巨细胞病毒(HCMV)感染的聚合酶链反应/逆转录—聚合酶链反应(PCR/RT-PCR)方法,并探讨糖尿病与HCMV感染的关系。方法在HCMV 65 kD低基质磷蛋白基因(pp65)内设计一对引物,建立PCR/RT-PCR法检测727例糖尿病患者及230名健康体检人员(对照组)外周血白细胞HCMV pp65 DNA及其mRNA转录水平,同时应用间接免疫荧光法分析外周血白细胞HCMV PP65抗原。结果糖尿病患者HCMVpp65 DNA,mRNA及PP65抗原阳性率分别为36.04%,16.64%和10.04%,与对照组4.78%,2.17%及1.30%相比,差异均有显著性(P<0.01)。发病年龄<30岁患者组中HCMV pp65 mRNA和PP65抗原阳性率明显高于其他年龄组(P<0.05)。病程<1年的糖尿病患者HCMV pp65 mRNA和PP65抗原阳性检出率最高,分别为27.54%与15.94%;而HCMV pp65 DNA则相反,其最高检出率分别是年龄≥60岁(42.19%)或病程≥15年患者(44.68%)。结论糖尿病患者易发生活动性HCMV感染,外周血白细胞pp65 DNA,mRNA及PP65抗原检测是临床快速诊断HCMV感染的有效手段,联合使用能更有效地提高诊断水平。Objective To establish polymerase chain reaction (PCR) and reverse transcriptase PCR (RT--PCR) for detecting human cytomegalovirus (HCMV) and to find out the relationship between diabetes mellitus (DM) and HCMV infection. Methods HCMV pp65-DNA and mRNA were simultaneously detected by PCR/RT-PCR using the primer sequences from HCMV pp65 genome in peripheral blood leucocyte of 727 patients with DM and 230 normal control. HCMV PP65-antigen in peripheral blood leucocyte from 957 samples were detected by indirect immunofluorescence assay. Results Positive rate of HCMV pp65-DNA, mRNA and PP65-antigen in 727 patients was 36. 04%, 16. 64% and 10. 04% respectively, there was significant difference compared with control group (HCMV pp65-DNA 4. 78%, ntR2qA 2. 17% and PP65-antigen 1.30%)( P〈0. 01). Positive rate of HCMV pp65- mRNA and PP65-antigen in patients whose age of disease onset under 30 years old was significant higher than persons in the other age group ( P〈0. 05). The highest positive rates of HCMV pp65 mRNA and PP65-antigen were in patients whose disease course was ( 1 years , which was 27. 54% and 15.94% respectively; The highest detection rate of HCMV pp65-DNA was found in the patients more than 60 years old (42. 19%) or patients whose disease course was ≥15 years (44. 68%). Conclusion Active HCMV infection can easily occurs in patients with DM, pp65-DNA, mRNA and PP65-antigen in peripheral blood leucocyte are useful for rapid detection of HCMV in clinics, combined assays may improve the diagnosis.
