辛伐他丁治疗12例小儿难治性肾病综合征脂质紊乱

摘　　要：目的:探讨辛伐他丁治疗小儿原发性难治性肾病综合征(SRNS)患儿高脂血症的疗效。方法:用酶法检测12例SRNS患儿用甲基羟戊二酰辅酶A(HMG-CoA)还原酶抑制剂辛伐他丁(舒降之)治疗8周前后血胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白AI(Apo AI)及载脂蛋白B(Apo B)水平,并设50例年龄、性别相匹配的健康儿童为对照组。结果:与治疗前相比,SRNS患儿治疗后血TC、TG、HDL-C、LDL-C及Apo B分别下降34.5%、38.3%、37.1%、39.5%(P<0.05),血HDL-C、Apo AI无明显变化。有1例患儿出现ALT轻度增高,在停药后恢复正常。结论:辛伐他丁可有效地改善SRNS脂质紊乱,但其长期效果及安全性需进一步研究。Objective.Simvastatin has a beneficial effect on abnormal lipid levels in children with SRNS, but the effectiveness of long - term therapy needs to be evaluated. To study the effect of simvastatin on children with steroid - resistant nephrotic syndrome（SRNS）. Methods. 12 children with SRNS treated 8 weeks with simvastatin which is a kind of hydrox- ymethylglutaryl CoA reductase inhibitor, were observed and 50 age - sex - matched healthy children as control. Six serum lipoprotein metabolism parameters including serum total - cholesterol （TC）, triglyceride （TG）, high - density lipoprotein cholesterol （HDL - C）, low - density lipoprotein cholesterol （ LDL - C ）, apolipoprotein AI （Apo AI）, apolipoprotein B （Apo B） were detected by enzyme method. Results： The significant reduction of serum TC, TG, LDL- C, ApoB in the treated group were 34.5 % ,38.3 % ,37.1% ,39.5 % respectively（P 〈 0.05）. And the levels of serum HDL C, ApoA1 had not significantly changed. There was a slight ALT abnormality in 1 cass, but it recovered soon after stopping using simvastatin. Conclusion： Simvastatin has a beneficial effect on abnormal lipid levels in children with SRNS, but the effectiveness of longterm therapy needs to be evaluated.
