摘 要:目的评价不同剂量、不同种类基因重组乙型肝炎(乙肝)疫苗对新生儿免疫后6年的免疫效果。方法连续对1996~1997年出生、母亲乙肝病毒表面抗原(HBsAg)阴性、接受过0、1、6个月程序基因重组乙肝疫苗免疫的218名新生儿队列进行追踪观察6年,采用血清流行病学方法,对接种重组乙肝疫苗[中国仓鼠卵巢细胞(CHO细胞)]和不同剂量重组乙肝疫苗(酵母)的3组儿童进行免疫效果比较。结果免疫初期(1~3年)重组乙肝疫苗(CHO细胞)的乙肝病毒表面抗体(抗-HBs)阳转率总体趋势优于重组乙肝疫苗(酵母)(Y0=6.150~11.57,P〈0.05),观察期间未发现HBsAg阳性者。免疫初期两种不同剂量重组乙肝疫苗(酵母)的免疫后抗体未显示出明显的差异。第6年随访了79名儿童,乙肝病毒核心抗体(抗-HBc)阳性者4名(占5.06%)。结论两种不同种类的基因重组乙肝疫苗对新生儿免疫后中期效果良好,母亲HBsAg阴性新生儿完成基因重组乙肝疫苗全程接种后,在6年的随访期问,不同疫苗组间抗体阳转率和抗体浓度下降趋势一致。Objective To evaluate the immunization effect of Hepatitis B gene recombined vaccine for the different doses and kinds of the vaccine during six years after the new infants were immunized. Methods The serum epidemiological survey was used in this study.218 new infants born in 1996 and 1997, whose mothers were HBsAg negative were followed for 6 years in this cohort study. All the infants were immunized by the Hepatitis B gene vaccine according to 0,1, and 6 immunization procedure and were divided into three groups. The immunization effect of CHO and YDV was analyzed and com- pared respectively. Results The anti-HBs positive rate of Chinese hamster ovary cell recombinant Hepatitis B vaccine (CHO) was superior to yeast recombinant Hepatitis B vaccine (YDV) in the initial immunizing stages(X^2 = 6. 150 - 11.57, P〈0.05) ,and there was not HBsAg positive. In the 6th year, 4 children were anti-HBc positive among 79 children. The anti-HBc positive rate was 5.06 96 . In the initial immunizing stages, there were no statistical differences between the two doses of YDV. Conclusions The immunization effects of the two kinds of Hepatitis B vaccines were good in the middle immunization stages. The anti-HBs positive rate and GMC were both descending gradually during the six years after the new infants were immunized completely.
