摘  要:目的比较微创经皮钢板内固定与传统内固定治疗胫骨远端骨折的疗效。方法自1998年10月至2002年5月,80例胫骨远端骨折患者随机分为2组,微创经皮钢板内固定组与传统切开复位内固定组各40例。结果术后平均随访20个月,微创经皮钢板内固定组中所有骨折均愈合,平均骨折愈合时间为11.9周。传统切开复位内固定组中,骨不连7例,伤口感染6例,钢板外露6例。结论微创经皮钢板内固定治疗胫骨远端骨折,骨折愈合快,感染率低,是1种较好的生物学固定方法。Objective To evaluate the effect of mlnimally invasive percutaneous plate osteosynthesis(MIPPO) for fractures of distal tibia and compare it with that of traditional low contact dynamic compressive plate (LC-DCP) osteosynthesis. Methods From October 1998 to May 2002,80 patients with distal tibial fracture were randomly assigned into MIPPO group and traditional group. The patients in MIPPO group were treated with minimally invasive pereutaneous plate osteosynthesis by indirect reduction under " C" - arm guidance, while the patients in traditional group were treated with open reduction and internal fixation of LC-DCP. Results At final follow-up (mean,20 months) ,all the fractures healed without second procedures and the mean union time was 11.9 weeks in MIPPO group. In the traditional group , the tibial nonunion were in 7 patients, infection in 6 patients, plate exposure in 6 patients. Conclusion Minimally invasive percutaneous plate osteosynthesis is thought to cause no increase in the risk of infection or soft tissue damage, and permits rapid bone union. It is a biological fixation method.
