髓内钉术后长骨干慢性骨髓炎手术方法的改进

摘　　要：目的探讨髓内钉术后长骨干并发症的防治方法。方法经采用骨皮质偏心置钉外固定架替换髓内固定，髓腔内置带侧孔连通管，术后冲洗，应用敏感抗生素等治疗，炎症消退后植骨。结果39例获得随访31例，伤口一次治愈29例，经换药后治愈2例，术后半年左右二期植骨，骨折在术后5～24个月，平均15．8个月内愈合，随访6～56个月内无复发。结论本法手术损伤轻，患者痛苦小，治疗彻底，术后恢复快，后续治疗简便，疗效满意。Objective To introduce a new plan to settle the problem of chronic osteomyelitis combined with non - healed fracture of long bone after the operation of bone nail in order to lessen the operative damage and to avoid bunging tube when douching after operation. Methods The nail of external fixator was screwed through the anteroposterior bone before taking it out, leading in the douche tube with lateral apertures through the hole through which the bone nail entered, leading it out by the windowing in the distal segment cortical bone, fixing the external fixator after the operation, douching the focus of infection with antibiotics. The operation of bone graft was done after the infection was controlled.Results Among 39 patients who were treated by the method,31 patients were followed-up,29 patients were cured once, 2 cases were cured after change dressings, bone grafted was done at about half a year after the operation, the fracture healed up at 5 - 24 months, mean 15.8 months. None of the 31 patients recurred during the period of following up for 6 - 56 months. Conclusion The injury of the surgical method is lighter, and the suffering of patients is slighter. The therapy is complete,with fast postoperative recovery and satisfactory therapeutic effects.
