卡托普利和缬沙坦对兔动脉粥样硬化腹主动脉超声表现的影响

摘  要:目的:超声下观察高脂饲养兔腹主动脉动脉粥样硬化(A S)斑块形成及内膜-中膜厚度(IM T),并观察卡托普利和缬沙坦干预后腹主动脉超声变化。方法:雄性健康新西兰兔随机分为高脂饮食组(1%胆固醇+4%猪油的高脂饮食);卡托普利干预组(高脂饮食、卡托普利每天2 m g/kg);缬沙坦干预组(高脂饮食、缬沙坦每天10 m g/kg);正常对照组(标准兔饲料),共喂饲10周。期间于0、6、10周末对兔腹主动脉行超声检查,记录IM T值。10周末处死动物取主动脉组织作病理形态学观察。结果:6、10周时高脂饮食组、卡托普利干预组、缬沙坦干预组与正常对照组相比血脂(血清总胆固醇、甘油三酯及低密度脂蛋白)水平显著升高(P<0.01),卡托普利和缬沙坦干预组与高脂饮食组相比血脂水平下降无统计学意义;超声检查:10周时卡托普利干预组和缬沙坦干预组与高脂饮食组相比IM T值显著降低(P<0.001)。高脂饮食组10周与6周IM T值相比显著增厚(P=0.049),卡托普利干预组、缬沙坦干预组IM T值10周与6周相比差异无显著性(P=0.111和P=0.285);10周时超声腹主动脉IM T值与病理学内膜厚度呈正相关关系(r=0.756,P<0.01)。结论:血管紧张素转换酶抑制剂及血管紧张素受体拮抗剂抑制A S的形成和发展;二维超声检测IM T值可准确反映A S病变情况。Objective:To observe the effect of angiotensin converting enzyme inhibition captopril and angiotensin Ⅱ type 1 receptor blockade valsartan on the formation of atherosclerosis and the intimal-media thickness(IMT) of abdominal aorta in atherosclerotic rabbits by ultrasonographic method. Methods :Male New Zealand white rabbits were randomly divided into cholesterol group;cholesterol with captopril group;cholesterol with valsartan group;and control group (normal rabbit chow). Ultrasonographic image was performed in 0,6,10 weeks respectively with HDI5000 and recorded the intimal-media thickness of abdominal aorta. After 10 weeks,the aorta was harvested for pathologic observation. Results:Captopril group, valsartan group and cholesterol group showed higher serum lipids levels than those of control group(P〈0.01 ). The lever of serum lipids were no difference among captopril group ,valsartan group and cholesterol group ; In 1 0 weeks , IMT of abdominal aorta in captopril group were significantly decreased ( P〈 0. 001 ) compared with cholesterol group and the same in valsartan group;In 10 weeks ,IMT were significantly higher (P=0. 049) than that in 6 weeks in cholesterol group. IMT of abdominal aorta in 10 weeks was no difference compared with that in 6 weeks of captopril group(P=0. 111) or valsartan group(P=0. 285);IMT of ultrasoncgrapby in abdominal aorta was positively correlated with the intimal thinkness of pathology(r = 0. 756,P〈0.01 ). Conclusion: The formation and the development of atherosclerosis could be inhibited by angiotensin converting enzyme inhibition captopril or angiotensin Ⅱ type 1 receptor blocked valsartan;The IMT of ultrasonograpby could accurately reflect the atherosclerotic lesion.
