0.5%左旋布比卡因与0.5%布比卡因用于硬膜外阻滞子宫切除术的麻醉效果比较

摘　　要：目的 比较0．5％左旋布比卡因和0．5％布比卡因在子宫切除术中硬膜外麻醉的临床效果。方法 60例子宫切除术病人随机分为两组，每组30例。均于T12～L1行硬膜外穿刺，向头侧置管3cm；下管取L3～L4行硬膜外穿刺，向尾侧置管3cm。于上管注入1％利多卡因5ml试验量，5min后注入首剂0.5％左旋布比卡因（A组）或布比卡因（B组）8～15ml，左旋布比卡因总量不超过200mg，布比卡因总量不超过100mg。结果 两组病人用药总量、肌松效果、感觉阻滞及VAS评分无显著差异，A组对血压、心率影响更小。结论 0.5％左旋布比卡因比0.5％布比卡因更适用于硬膜外麻醉。Objective TO compare the clinical effects of 0. 5% levobupivacaine and 0. 5% bupivacaine for epidural anesthesia in total hysterectomy. Methods Sixty patients （ASA I - Ⅱ ） were divided into two groups randomly （n = 30 in each group）. In all patients epidural space T12-L1 and L3-4 were selected, and the catheter was put upward and downward 3 cm respectively. 1% Lidocaine （ 5 ml） was used as a test dose and 5 rain later, the initial dose of levobupivacaine （Group A） or bupivacaine （group B） 8 - 15 ml was injected. The total dose of Levobupivacaine was less than 200 nag and that of Bupivacaine less than 100 nag. Results There was no significant difference in total dose, level and onset time of sensory block and score of visual analog scale （VAS）, analgesia and muscle relaxation between two groups. In group B, 10 rain following infusion, the blood pressure was decreased （P〈0. 05）, Conclusion 0. 5% Levobupivacaine was more suitable for epidural anesthesia than 0. 5% Bupivacaine.
