悬吊术治疗压力性尿失禁的临床研究

摘　　要：目的探讨经阴道经闭孔尿道中段补片悬吊术治疗压力性尿失禁的疗效。方法对压力性尿失禁患者分两组进行治疗:采用经阴道无张力尿道中段悬吊术治疗组23例;采用经阴道经闭孔尿道中段补片悬吊术治疗组32例。结果55例患者均有效。经阴道无张力尿道中段悬吊术组治愈率82.6%,改善率17.4%;经阴道经闭孔尿道中段补片悬吊术组治愈率96.9%,改善率3.1%,经阴道无张力尿道中段悬吊术和经阴道经闭孔尿道中段补片悬吊术两组平均手术时间分别为54.7m in和27.4m in(P<0.01),平均住院日分别为6.1天和3.2天(P<0.01),平均置留尿管时间分别为1.2天和5.8小时;平均医疗费用分别为8 652元和1 247元(P<0.01)。结论经阴道经闭孔尿道中段补片悬吊术治疗压力性尿失禁手术简单、安全、费用低廉,是治疗压力性尿失禁可推广的一种手术方式。Objective To investigate therapeutic effect of tension-free vaginal tape 2 obt urator （TVT20） for female stress urinary incontinence （SUI）. Methods The patients with SUI were divided into two groups. The patients in the group A （23cases） were treated with TVT and those in the group B （32 cases） were treated with TVT-20. Results For all 55 cases, the two treatment methods were effective. The cure rate in the group A was 82.6% and the remission rate was 17.4%. The cure rate in the group B was as high as 96.9% and the remission rate was 3.1%. The average operation time in the group A and the group B was 54.7 min. and 27.4 min. , respectively （P 〈 0.01 ）. The average length of hospital stay in the group A and the group B was 6.1 days and 3.2 days, respectively （ P 〈 0.01 ）. Urine tubes were remained for 1.2days and 5.8 hours respectively. The average cost of operation was 8,652 RMB and 1,247 RMB respectively （P 〈0.01 ）. There were 3 cases of urine retention in the group A and 2 cases in the group B. No urethrovaginal fistula, bladder injury or other serious complications were found in either group A or group B. Conclusion TVT-20 is a simple, safe, inexpensive method for treatment of SUI and is worthy to be popularized in clinic.
