摘 要:目的检测从ICU患者痰液标本中分离的产MBL、ESBL脑膜炎败血黄杆菌的耐药性状况,指导临床正确合理使用抗生素。方法用VITEK32鉴定菌株;双纸片协同试验筛选产MBL菌株、产ESBL菌株;药敏试验使用VITEK GNS143上机检测和K-B法检测加做的药敏试验。结果ICU患者痰液标本中的脑膜炎败血黄杆菌的产酶率为78.4%,其中产MBL为41.2%,产ESBL为29.4%,有4株同时产MBL和ESBL。美满霉素、万古霉素、力奈唑烷的耐药率分别为19.8%、1.98%、1.98%。结论脑膜炎败血黄杆菌在ICU患者的痰液标本中高耐药性的根本原因是高产酶率(高产MBL和ESBL)。万古霉素、力奈唑烷、美满霉素是治疗ICU患者呼吸道感染的最有效药物。Objective To investigate drug resistance of Chryseobacterium meningosepticum which producing metallo-β-lactamase and extended-spectrum-β-lactamase from ICU patients' specimen of sputum so as to guide the rational application of antibiotics. Methods Identified the strains with VITEK 32 ; MBL was screened by double disk synergy and ESBL was screened by double disk synergy; Antimicrobial sensitivity of clinical isolated was tested by VITEK GNS143 and adding antimicrobial sensitivity was tested by Kirby-Bauer method. Results The rate of producing β-lactamase of Chryseobacterium meningosepticum was 78.4%,and the rate of producing MBL was 41.2%, the rate of producing ESBL was 29. 4%,respectivily, four clinical isolates produced both MBL and ESBL. The resistance rate of Chryseobacterium meningosepticum clinical isolates to Minocycline, Vancomycin, Linezolid was 19.8%, 1.98%,1.98%. Conclusions Why the high resistance rate of Chryseobacterium meningosepticum in ICU patients' specimen of sputum was that high producing β-lactamase(high producing MBL and ESBL). Vancomycin, Linezolid, Minocycline were the most effective agents for treatment in ICU patients' specimen of sputum.
