急性冠脉综合征患者在不同活动时心肌耗氧量变化的研究

摘　　要：目的观察急性冠脉综合征患者在不同活动时心肌耗氧量的变化。方法分别测量患者排便、听音乐、接受探视时,以及接受医护处置前后心率和血压变化情况,并应用二项乘积方法计算心肌耗氧量。结果在排便时心肌耗氧量由(82.1±17.8)×102增加到(122.4±40.4)×102,有显著性差异(t=3.610,P<0.05)。接受探视时心肌耗氧量由(83.3±17.5)×102增加到(104.8±33.7)×102,有显著性差异(t=3.048,P<0.05)。在医护处置情况时心肌耗氧量由(82.3±16.3)×102增加到(99.8±52.7)×102,有显著性差异(t=2.940,P<0.05)。听音乐时心肌耗氧量由(87.3±10.8)×102增加到(97.4±28.2)×102,无显著性差异(t=0.996,P>0.05)。结论急性冠脉综合征患者在排便、接受探视和接受医护处置时心肌耗氧量明显增加,提示在护理实践中,要保持患者大便通畅,尽可能减少探视和医护处置的频率。Objective To observe the change of myocardial oxygen consumes quantity in different activities of the patients with acute coronary syndrome. Methods The patients＇ heart rate and blood pressure were measured before and after the activities such as defecation, listening to music, guest visiting and receiving medical treatment. And the double-product （D-P） were calculated to represent the myocardial oxygen consume quantity. Results The average D-P was significantly increased from （82.1±17.8）×10^2 to （ 122.4±40.4）×10^2 （t = 3.610, P〈0.05） in the period of defecation. The average D-P when guests visited was also significantly increased from （83.3±17.5） ×10^2 to （104.8±33.7）×10^2 （t = 3. 048, P〈0.05）. During the medical treatment, the D-P changed from （82.34±16.3）×10^2 to （99.8±52.7）×10^2 ,and the difference was significant （t = 2. 940, P〈0.05）. During listening to music, the D-P changed from （87.3±10.8） ×10^2 to （97.4±28.2） ×10^2 ,and the difference was not significant （t=0.996,P〈0.05）. Conclusions In patients with acute coronary syndrome, the myocardial oxygen consume quantity was significant increased in the period of defecation, guest visiting and medical treatment. It is suggested that keeping defecation normal, reducing frequency of guest visiting and medical treatment be important in nursing.
