子宫内膜病变中AgNOR与C-erbB-2蛋白表达关系的研究及临床意义

摘　　要：目的研究核仁组成区嗜银蛋白(AgNOR)与C-erbB-2蛋白在子宫内膜病变中的表达规律及二者关系。方法用银染法、免疫组化S-P法检测正常子宫内膜25例作为对照组,子宫内膜增殖症25例、非典型增生19例、子宫内膜腺癌高分化26例、中分化18例、低分化19例,共107例作为观察组,观察在正常子宫内膜及不同内膜病变中AgNOR的计数值和C-erbB-2蛋白表达情况以及两者的关系。结果AgNOR计数在子宫内膜腺癌中升高,与对照组和子宫内膜良性病变组比较有统计学意义(P<0.05),在高、中、低分化组中的计数值分别为(4.69±2.32)、(5.38±2.99)、(7.54±2.09),低分化组与高分化组比较有统计学意义(P<0.05);AgNOR计数在非典型增生和子宫内膜增殖症组中虽升高,与对照组比较无统计学意义(P>0.05);C-erbB-2蛋白在子宫内膜腺癌中阳性表达率明显升高,与对照组和子宫内膜良性病变组比较有统计学意义(P<0.05);二者存在正相关(r=0.854,P<0.05)。结论AgNOR和C-erbB-2蛋白与子宫内膜腺癌的发生密切相关,AgNOR计数增高与C-erbB-2蛋白的表达存在正相关。AgNOR值增高提示患者可能患有子宫内膜腺癌且提示子宫内膜腺癌恶性程度高,如合并C-erbB-2蛋白的阳性表达率增高可进一步提示患者患有子宫内膜腺癌。[Objective] To investigate the expression and clinical significance of AgNOR and the correlation of AgNOR and C-erbB-2 in endometfial diseases. [Method] Experimental group： 132 cases including 63 of endometrial carcinoma, 25 of endometrial hyperplasy, 19 of atypical proliferation. Control group： 25 cases of normal endometrium. Count AgNOR in every nuclear at random and detect the correlation of AgNOR and protein C-erbB-2 in these cases. [Results] Data of AgNOR in endometrail carcinoma group have statistical significance compared with the control group and hyperplasy or atypical groups （P 〈0.05）. The expression of protein C-erbB-2 obviously in- crease in endometrial carcinoma and it is significant （P 〈0.05）, AgNOR is correlated with C-erbB-2 （r=0.854, P 〈 0.05）. [Conclusion] AgNOR and C-erbB-2 is related to the incidence of endometrial carcinoma. The high scores of AgNOR and the positive of C-erbB-2 indicates a high degree of malignance of patients with endometrial carcinoma.
