畸形子宫35例分析

摘　　要：目的探讨各种畸形子宫的临床特征、诊断、处理原则及与不孕症的关系。方法对2002年3月～2005年10月该中心收治的35例住院行腹腔镜术(宫腔镜)/开腹手术确诊为畸形子宫患者的临床资料进行回顾性分析。结果35例患者既往宫内妊娠率为37.14%,其中残角子宫正常妊娠率为50.00%,子宫不全中隔为14.29%,完全中隔为57.14%;双角子宫2例均为原发性不孕症患者;双子宫3例,1例有足月产史,2例为原发性不孕症;完全中隔子宫中有自然流产史者占50.00%(2/4);B超诊断畸形子宫符合率为28.57%,但对完全中隔子宫诊断符合率较高85.71%(6/7),子宫输卵管造影(HSG)诊断符合率极低;畸形子宫合并子宫内膜异位症(EMT)患病率为51.43%,手术后妊娠率为27.78%。结论残角子宫、完全中膈子宫、双子宫均有一定的妊娠机会,但完全中膈妊娠易自然流产,双角子宫、子宫不全中膈患者妊娠几率低。该文病例手术后妊娠率无提高。[Objective] To improve the cognition of each kind of malformed uterus＇ clinical characteristics, the diagnosis, the processing principle and to explore the relationship between the malformed uterus and sterility. [Methods] A retrospective analysis was conducted on 35 female sterility patients who were diagnosed or treated through laparoseopy ete in our center from March 2002 to October 2005 [Results] Among 35 patients, the previous pregnancy rate was 37.14%, and rudimentary horn of the uterus was 50.00%, the partial septate uterus was 14.29%, entire septate uterus was 57.14%. Both of the two bicomuate uterus patients were primary sterility. In these three didelphie uterus patients, two of them were primary sterility, the other one had a history of full-term pregnancy. Patients with entire septate uterus had a abortion radio of 50.00%（2/4）. Transvaginal ultrasound had a 28.57% diagnastie accordance rate on malformed uterus, however, on the entire septate uterus, it was 85.71%（6/7）. There was a lowest diagnostic accordance rate on malformed uterus by hysterosalpingngraphy （HSG）. 51.43% of these 35 patients were diagnosed with endometriosis. The pregnancy rate was 27.78% after operation. [Conclusions] There is a eerlain pregnancy opportunity among these patients with rudimentary horn of the uterus or didelphie uterus or entire septate uterus, but a lower pregnancy rate of these patients with bieornuate uterus or partial septate uterus. Entire septate uterus have a high abortion radio. The pregnancy rate has not been improved after the treatment by surgery.
