初发2型糖尿病并非酒精性脂肪肝与代谢综合征相关分析

摘　　要：目的探讨初发2型糖尿病并非酒精性脂肪肝(简称脂肪肝)与代谢综合征的关系。方法采用病例对照研究,新发2型糖尿病伴有脂肪肝(DFL)组66例,新发2型糖尿病不伴脂肪肝(NDFL)组49例,检测两组的体重指数(BMI)、腰围(WC)、腰臀比(WHR)、血压、血脂、血尿酸、空腹血糖、空腹C肽和餐后2hC肽,采用空腹C肽代替胰岛素改良HOMA公式评价胰岛素抵抗。结果DFL组与对照组比较,DFL组体重指数、腰围、腰臀比、甘油三酯(TG)、血尿酸和空腹C肽均具非常显著升高(P<0·01);舒张压(SBP)、餐后2hC肽和HOMA-IR(CP)均明显升高(P<0·05);两组空腹血糖水平差异无统计学意义;脂肪肝组代谢综合征的发生率明显高于对照组。结论初发2型糖尿病合并脂肪肝存在明显胰岛素抵抗;脂肪肝可能是代谢综合征的一个组成部分。Objective To investigate the relationship of nonalcohoLic fatty liver disease （called ＂fatty Liver＂ for short） with newly - diagnosed type 2 diabetes patients and metabolic syndrome（MS）. Methods The investigation was conducted by means of case control study, including 66 patients of newly - diagnosed type 2 diabetes with fatty Liver （DFL）, 49 newly - diagnosed type 2 diabetes without fatty Liver （NDFL）. Body mass index （BMI）,waist circumference（WC）, waist to hip ratio（WHR）, blood pressure, serum Lipids, serum uric acid, fasting plasma glucose, fasting C - peptide and 2 - hour C - peptide were measured. Fasting C - peptide was used to replace insulin in homeostasis model assessment （HOMA） for assessment of the status of insulin resistance. Results BMI, WC, WHR, triglyceride, serum uric acid and fasting C - peptide was higher in patients with fatty liver in newly - diagnosed type 2 diabetes than those of the controls （P 〈 0.01 for all）. Diastolic pressure, 2- hour C - peptide and HOMA - IR（CP） were significandy higher in the patients of fatty Liver group （ P 〈 0.05 for all）. There no significant difference in the levels of fasting plasma glucose was observed in the two groups. The incidence of metabolic syndrome in the patients of fatty Liver group were significandy higher than those in the controls. Conclusion There severe insulin resistance is observed in patients with fatty Liver in newly - diagnosed type 2 diabetes; Diabetic fatty liver probably is one of components of metaboLic syndrome.
