双腔T管在胆道手术中的应用体会

摘　　要：目的 探讨双腔T管在胆道手术中的应用效果。方法 2001年5月～2005年10月对25例胆道手术患者采用双腔T管行胆道冲洗、肠内营养及用作胆管支架。结果 全组未出现胆道逆行感染、胆瘘等严重并发症。1例冲洗时出现轻度腹痛，1例出现外管堵塞，及时处理后解除。22例于术后半月拔除双腔T管痊愈出院；3例严重肝破裂胆管损伤患者中，2例于术后半年拔管，分别随访1年及8个月无异常，另1例术后带管出院4个月，尚在随访中。结论 双腔T管具有引流、冲洗、肠内营养及留置支架等多种用途，取材容易，操作简便，值得推广。Objective To study the effect of double-lumen T-tube on surgery for biliary tract. Method 25 cases received the double-lumen T-tube as an approach for flushing, enteral nutrition and stenting. Result No retrospective infection and leakage occurred. 1 case experienced mild abdominal pain, 1 case experienced tube occlusion that were treated responsively. T-tubes were removed in 22 cases within 15 days postoperatively. The tubes stayed longer in 3 cases of traumatic liver rupture with hepatic ductal injury, specifically, 6 months in 2 cases and 4 months in 1 case subjected to further observation. Conclusion Double-lumen T-tube can be mutiple functioned as draining, flushing, nutrition feeding and stenting.
