三环疗法治疗慢性乙型病毒性肝炎肝纤维化40例疗效观察

摘　　要：目的 观察三环疗法治疗慢性乙型病毒性肝炎肝纤维化的疗效并探讨其作用机制。方法 全部80例．随机分为2组。治疗组40例足三里、阳陵泉、曲池穴位注射丹参注射液各3ml；健肝穴或三阴交穴穴位发泡；联合三环益肝丸（A2）3．5g,三环益肝丸Ⅱ（A2a）2、5g、三环保肝胶囊（A1）2粒，均每日2～3次口服。对照组40例予甘利欣胶囊3粒，复方丹参片4片，均每日3次口服。2组疗程均为6个月。观察2组治疗后症状改善情况、肝功能及血清透明质酸（HA）、Ⅲ型前胶原肽（PⅢP）、层粘连蛋白（LN）、Ⅳ型胶原（C-Ⅳ）、B超影像改善情况。结果 治疗组症状消失率优于对照组（P〈0．05），其肝功能复常和HA、PⅢP、LN、C-Ⅳ改善及B超影像的改善均优于对照组，2组比较有显著性差异（P〈0．01）。结论 三环疗法能明显改善症状，有一定保肝降酶、抗纤维化功效，且毒副作用小，使用范围广，可长期应用。Objective To study the clinical effect and mechanism of Sanhuan therapy on fibrosis of chronic hepatitis B. Methods 80cases of chronic hepatitis B were randomly divided into treatment group （docases） and control group （40 cases）. The cases in treatment group were treated by Sanhuan therapy for 6 months：Taken orally with Sanhuanyigan bills, Sanhuanyigan 11 bills and Sanhuanbaogan capsules;Treated with injection of Danshen injection 3 ml and Caihu injection 2 ml into Zusanli Point and Yanglingquan Point ; Apllied with vesicant 3 mg to Jiangan Point and Sanyinjiao Point . the control group was treated by Ganlixin capsules and Fufangdanseng tablets. The parameters of liver function, fibrosis and B X-ray of patients were observed in both groups. Results After treatment, the level of hyaluroinc acid （HA） decreased significandy （97.3 ± 17.5 vs. 132.4 ± 19.9）, and level of leminin （LN） （75.1 ± 5.9VS 112.9 ± 19.0）and level of type Ⅳ collagen （ C-Ⅳ ） （81.3 ± 22.4 vs. 126.9 ± 41.2）decreased significantly too（ all P 〈 0.01 ） ; efficiency on B X-ray improved 78.6% vs. 52.4% （ P 〈 0.05）. Treatment group was superior to control group in promoting symptoms and lowering indexes of liver inflammation and fibrosis, and B X-ray improve significandy. Conclusion The results showed Sanhuan therapy can improve the liver inflammation, liver fibrosis and main symptoms, and can reduce side effect.
