瑞芬太尼与芬太尼复合麻醉用于支撑喉镜的对比观察

摘　　要：目的:观察瑞芬太尼复合丙泊酚麻醉对术中病人的血流动力学的变化,及停药后病人自主呼吸恢复时间、清醒拔管时间。方法:40例声带息肉,声带小结的病人随机分为两组,R组为瑞芬太尼复合丙泊酚组;F组为芬太尼复合丙泊酚组。两组于麻醉诱导后,行气管插管后,手术开始时,记录病人的MBP、HR,及术毕停药至自主呼吸恢复时间、清醒拔管时间。结果:两组血压、心率变化不大,无统计学差异(P>0.05),R组病人恢复时间、清醒拔管时间比F组明显提前,有显著性差异(P<0.05)。结论:瑞芬太尼复合丙泊酚麻醉能明显缩短病人清醒拔管时间。Objective： To observe the hemodynamic change, recovery time of autonomous respiration after stopping the anesthesia and the decannulation time with consciousness for the patients who are practiced the compound anesthesia with remifentanil and propofol during the operation. Method： A total of 40 cases patients with polyp of vocal cord or vocal nodules was divided into group R and Group F. Remifentanil compounding with propofol was used in group R and fentanil compounding with propofol was used in group F. After the anesthesia induction and tracheal intubation, the MBP and HR of the patients were recorded when the operation was started. And the recovery time of autonomous respiration and decannulation time with consciousness also were recorded after the operation. Result： The changes of blood pressure and heart rate were not obvious in two groups. There was no statistically different in two groups （ P 〉 0.05 ）. The recovery time of autonomous respiration and the decannulation time with consciousness in Group R were much shorter than those in Group F. The difference is obvious （ P 〈 0.05 ）. Conclusion： The compound anesthesia with remifentanil and propofol can obviously shorten the decannulation time of patient with consciousness.
