^(99)Tc^m-MIBI心肌灌注显像半定量评价心肌梗死后左室重构

摘　　要：目的应用99Tcm-甲氧异丁基异腈(MIBI)心肌灌注显像对左室重构进行半定量评估。方法随机从99Tcm-MIBI心肌灌注显像库中抽取心肌梗死患者85例,其中前壁组25例,下后壁组30例,复合壁组30例。另取正常对照组30例。采用自编软件测定前壁或下后壁梗死部位局部膨出指标EG/EF或FG/EF值,左室心肌梗死后球形化指标EF/AB值。结果前壁组、复合壁组EG/EF值明显高于正常组(P<0.01),复合壁组FG/EF值明显低于正常组(P<0.01);患者组EF/AB值均明显高于正常组(P<0.01),其中前壁组、复合壁组EF/AB值明显高于下后壁组(P<0.01),前壁组、复合壁组的EF/AB值无显著性差异(P>0.05)。结论EG/EF、EF/AB等指标可用于心肌梗死后左室重构的半定量评估。Objective To quantitatively evaluate left ventricular remodeling（LVRM） using ^99Tc^m- MIBI myocardial SPECT imagingo Methods Eighty five patients randomly sampled from ^99Tc^m- MIBI myocardial SPECT imaging database , were divided into three groups according to different infarction loeums： Twenty five patients with interior infarction, 30 patients with inferoposterior and 30 patients with interior and inferoposterior. Thirty volunteers were enrolled in this study as the normal controls. The value of EG/EF or FG/EF and the value of EF/AB were measured by a program devel- oped ourselves. Results The value of EG/EF in patients with interior infarction and in patients with interior and inferoposterior infarction were significantly higher than those in the normal group, the values of FG/EF in patients with interior and inferoposterior infarction were significantly lower than those in the normal group. The value of EF/AB in patients were significantly higher than those in the normal group, and in patients with myocardial infarction, the values in the interior wall group and the interior and inferoposterior group were significantly higher than those in the inferoposterior group, and there was no significant difference between the interior wall group and inferoposterior wall group. Conclusions EG/EF and EF/AB may be used to quantitatively evaluate LVRM after myocardial infarction.
