充血性心力衰竭患者Tei指数的变化及其临床意义

摘　　要：目的探讨充血性心力衰竭(congestive heart failure,CHF)患者Tei指数的变化及其临床意义。方法54例充血性心力衰竭患者,按照其入院时脑钠素(BNP)的水平分为:BNP<1000 pg/ml的A组(33例)和BNP≥1000 pg/ml的B组(21例)。29例健康体检正常者作为C组。行超声心动图检查测量左室收缩末期内径(LVDs)、左室舒张末期内径(LVDd)、跨二尖瓣舒张早期血流速度与二尖瓣环舒张早期运动速度的比值(E/Ea)和Tei指数。结果(1)A、B两组的LVDs和LVDd和E/Ea较C组增大有统计学意义(皆P<0.05)。(2)A、B两组的Tei指数(0.72±0.11和0.95±0.16)较C组(0.44±0.07)显著性延长(P<0.05);B组与A组比较,Tei指数的延长更显著(P<0.05)。结论在心律规则的条件下,Tei指数能综合评价CHF患者的心功能。Objective To investigate the change of Tei index in patients with congestive heart failure. Methods Fifty four patients with congestive heart failure were divided into two groups according to brain natriuretic peptide （BNP） ： one group with BNP〈 1000 pg/mi（group A, n = 33）;the other with BNP≥1000 pg/ml（group B, n = 21）. The control group（group C） consisted of 29 normal subjects. The systolic and diastolic diameters of left ventricle （LVDs, LVDd）, the ratio of mitral peak velocity of early filling to early diastolic mitral annular velocity （E/Ea）and Tei index were measured by echocardiography. Results （1） LVDs, LVDd and E/Ea were increased in the group A and group B compared with group C（ P 〈 0.05）. （2） Tei indexes in the group A and group B were longer than those in the group C （0.72± 0.11, 0.95±0.16 vs 0.44± 0.07 , P 〈 0.05）, and Tei indexes in the group B were much longer than those in the group A （P 〈 0.05）. Conclusions Tei index was a valuable indicator for estimating left ventricular systolic and diastolic functions in patients with congestive heart failure.
