Influential factors related to metabolic syndrome on the outcome of non-diabetic subjects in a community of Shanghai by two-year follow-up

摘  要:目的探讨代谢综合征(MS)相关因素对社区非糖尿病人群二年转归的影响。方法对2002年上海市平凉社区糖尿病流行病学调查人群中的885名非糖尿病者进行2年随访,作75g葡萄糖耐量试验,测量血总胆固醇和甘油三酯(TG)、血压、身高、体重、腰围、臀围。结果(1)基线时糖调节受损(IGR)人群体重指数(BMI)、收缩压、舒张压、空腹血糖(FPG)、糖负荷后2h血糖(2hPG)、TG水平明显高于正常糖调节组(NGR)(均P<0.05);(2)与转归为IGR者和仍为NGR者相比,转归为糖尿病者基线时MS各组分水平已明显升高(P<0.05或P<0.01);(3)NGR、IGR人群糖尿病年转化率分别为0.95%、4.17%;(4)BMI≥25kg/m^2,发生糖尿病的风险为2.29倍(P<0.05);FPG、2hPG每上升1mmol/L,发生糖尿病的风险增加分别为4.66倍(P<0.01)、1.25倍(P<0.05)。结论MS组分中肥胖对非糖尿病人群转归糖尿病起重要作用,FPG升高者转归糖尿病的风险较大。Objective To investigate the influence of factors related to metabolic syndrome ( MS ) on the outcome in subjects without diabetes mellitus in a community. Methods A two-year follow-up study was conducted in 885 subjects who were enrolled in the epidemiologic survey carried out in Pingliang Community, Shanghai in 2002. Oral glucose tolerance test, lipid profile, blood pressure (BP) , body mass index (BMI) , waist and hip circumferences were measured. Results ( 1 ) The baseline of BMI, fasting plasma glucose (FPG) , 2 h plasma glucose after glucose loading (2hPG), BP, triglyceride (TG) in the subjects with impaired glucose regulation (IGR) increased significantly as compared to those with normal glucose regulation (NGR) (all P 〈 0.05 ) ; (2) In the diabetic subjects who were not diabetic at the beginning, the levels of the factors of MS at the baseline were significantly higher than those in IGR group and NGR group ( P 〈 0.05 or P 〈 0.01 ) ; ( 3 ) The annual cumulative incidence of diabetes in subjects of NGR and IGR were 0.95% and 4.17%, respectively; (4) In subjects with BMI≥25 kg/m^2 , the risk of diabetes was increased 2.29 folds ( P 〈0.05 ). The risk of diabetes increased 4.66 and 1.25 folds ( both P 〈 0.05 ) along with respective increase of FPG and 2hPG hy every 1 mmol/L. Conclusion Obesity plays an important role in the outcome of subjects without diabetes. Raised FPG is a significant risk factor of diabetes in the subjects studied.
