摘 要:目的:了解抗菌药物在胰腺的渗透率,为防治继发胰腺感染抗菌药物的选择提供依据。方法:通过35例胰十二指肠切除患者术中切除的胰腺组织和术后胰管内置管获得的纯胰液,用高效液相色谱法分别测定丁胺卡那、氨苄青霉素、头孢哌酮、头孢曲松、泰能、环丙沙星和甲硝唑在血清、胰组织和胰液中的浓度。结果:7种抗菌药物的平均胰液渗透率是:丁胺卡那为13%、氨苄青霉素为15%、头孢哌酮为17%、头孢曲松为25%、泰能为35%、环丙沙星为44%、甲硝唑为52%。7种抗菌药物的胰液和胰组织渗透率无显著差异(t检验,P>0·05)。结论:各种抗菌药物的胰腺渗透率差别较大,防治继发胰腺感染宜选用胰腺渗透率强的抗菌药物。Objective: To investigate penetration of antimicrobial dugs into human pancreas in order to provide information for choice of antimicrohial dugs in the prevention and treatment of secondary pancreatic infection. Methods: 35 cases of patients who underwent pancreatoduodenectomy were included in this study. The pancreatic tissues were obtained intraoperatively and pancreatic juice was collected via pancreatic duet catheterization extracorporally. The concentration of amikacin, ampicillin, cefoperaxone, Ceftriaxone, imipenem, ciprofloxacin and metronidazol in serum, pancreatic tissue and pancreatic juice were determined by high -performance liquid chromatography. Results: The permeability of amikacin, ampicillin, cefoperaxone, Ceftriaxone, imipenem, ciprofloxacin and metronidazol in pancreatic juice was 13%, 15%, 17%, 25%, 35%, 44% and 52%, respectively. The permeability of those 7 antimicrobial dugs in pancreatic juice and pancreatic tissue was not different significantly. Conclusion: The permeability of those 7 antimicrobial dugs in pancreas was various significantly. Obviously, the antimicrobial dugs with higher permeability should be chosen to orevent and treat secondary pancreatic infections.
