摘 要:目的观察不同潮气量机械通气对大鼠肺组织内毒素CD14受体表达的影响。方法成年雄性SD大鼠30只,体重200~300 g,随机分为3组(n=10):正常对照组(C组)、小湖气量机械通气组(S组)和大潮气量机械通气组(L组)。S组潮气量(V_T)8 ml/kg,N组V_τ40ml/kg;吸:呼比均为1:1;调整呼吸频率60~80次/min,维持呼气末二氧化碳分压在35~45 mm Hg。分别于机械通气前(基础值)、机械通气1、2、3 h行动脉血气分析,机械通气3h时放血处死大鼠,采用逆转录聚合酶链反应测定肺组织内毒素CD14受体mRNA表达;用免疫组织化学法测定左肺支气管灌洗液内毒素CD14受体蛋白表达。结果与C组比较,L组机械通气1h时动脉血pH值、氧分压升高,动脉血二氧化碳分压降低,肺组织内毒素CD14受体蛋白及mRNA表达均升高(P<0.01)。结论大潮气量机械通气3 h可上调大鼠肺组织内毒素CD14受体表达。Objective To investigate the effect of mechanical ventilation with different tidal volume on CD14 receptor in lung tissue. Methods Thirty adult male SD rats weighting 200-220 g were randomly divided into 3 groups (n= 10 each) : group Ⅰ received no mechanical ventilation (control) ; group Ⅱ received small tidal volume (VT = 8 ml·kg^-1) (S) and group Ⅲ received large tidal volume (VT = 40 ml·kg^-1 ) (L). The animals were anesthetized with intraperitoneal 20% urethane, tracheostomized and mechanically ventilated (I:E = 1 : 1, FiO2 = 0.21). PET O2 was mechanical at 35-45 mm Hg by adjusting the respiratory rate. The animals were killed by exsanguination after 3 h mechanical ventilation. The lungs were removed for determination of CD14 receptor mRNA expression in lung tissue (by RT-PCR ) . The left lung was lavaged and the expression of CD14 protein in macrophage collected from broncho-alveolar fluid (BALF) was determined by immunohistochemistry. Results The expression of CD14 receptor mRNA in lung tissue and CD14 receptor protein in macrophage collected from BALF were significantly increased in group Ⅰ as compared with control group ( P 〈 0.01 ) . Conclusion Mechanical ventilation with large VT for 3 h induce up-regulation of CD14 receptor expression in lung tissue.
