摘  要:目的:根据基础和临床两方面总结近年皮肤瘢痕的研究与治疗进展,认识其在治疗中的应用情况。资料来源:应用计算机检索Medline1987-01/2003-12有关皮肤瘢痕方面的文献,检索词“keloid,hypertrophicscar,treatment,keloidfibroblastsinvitro”,限定文章语言种类为English。同时计算机检索中国期刊网全文数据库1999-01/2003-12相关皮肤瘢痕基础研究和治疗的文章,检索词“瘢痕疙瘩,增生性瘢痕,成纤维细胞”,限定文章语言种类为中文。资料选择:对资料进行初审,选取包括皮肤瘢痕的相关文献,开始查找全文。纳入标准:与瘢痕基础研究和治疗有关的文献。排除标准:文献中重复研究,综述类文章。资料提炼:共检索到关于皮肤瘢痕的文献178篇,13篇文献符合纳入标准。资料综合:文章根据皮肤瘢痕的特点从实验研究和防治两方面对皮肤瘢痕的文章进行概括和评价。实验研究主要从细胞培养检测、补充元素、动物模型等方面进行概括。防治则从手术治疗、非手术治疗两方面进行概括。结论:皮肤瘢痕治疗主要是抑制成纤维细胞的蛋白合成,刺激蛋白酶特别是胶原酶的合成。以往国内各地自研用于临床的促愈合防治瘢痕药物存在缺乏系统基础研究、组方随意性大、缺乏科学依据和严谨疗效评价的问题。国外则多为实验室基础研究,实际开发出的临床应用产品不多。瘢痕目前尚未有特效的药物及治疗方法,其基础及临床研究有待进一步拓展。OBJECTIVE: To summarize the progress in research and treatment of skin scar in the recent years from foundation and clinic, and know its application in the treatment. DATA SOURCES: Articles on skin scar published from January 1987 to December 2003 were searched in Medline by computer with the key words of "keloid, hypertrophic scar,treatment, keloid fibroblasts in vitro" and the language was limited to English. Meanwhile, relevant Chinese articles published from January 1999 to December 2003 were searched in Chinese periodical full-text database with the key words of "keloid, hypertrophic scar, fibroblasts". STUDY SELECTION: Data were checked primarily, and articles related to the scar of skin were selected and looked for the full text. Inclusion criteria: Literatures related to the basic research and treatment of skin scar. Exclusion criteria: repetitive studies and reviews. DATA EXTRACTION: 178 articles about skin scar were collected, in which 13 articles were in accordance with the criteria. DATA SYNTHESIS: The study summarized and evaluated the characteristics of skin scar from both aspects of experimental study and treatment. Detection in cultivated cells, supplementary elements and animal models were generally described in experimental studies. The treatment section included surgical and non-surgical treatments. CONCLUSION: Treatment of skin scar is mainly inhibiting the synthesis of fibroblastic protein and stimulate the synthesis of protease, especially collagenase. Domestically, non-research bases, unreasonable regiments used in clinic patients are common and no restrictive evaluation criteria for treatment effect and randomize clinical trials are the major problems. Therapeutic prevention and treatment of skin scar are beyond serial and had less theoretical bases from science research.
