摘  要:目的研究不同肾素、血管紧张素II和醛固酮(RAS)水平对持续性心房颤动转复的影响。方法对持续性心房颤动患者,在使用口服胺碘酮转复后依转复效果分为转复成功组(n=19)和转复未成功组(n=17),比较两组患者血浆RAS浓度水平。进一步分析其浓度水平与左心房大小的相关性。结果在转复成功组患者血浆RAS水平显著低于转复未成功组(P<0.01、P<0.05);血浆RAS水平与左心房直径呈显著正相关(P<0.01)。结论对血浆RAS水平升高的持续性心房颤动患者,胺碘酮转复不易成功。AIM To probe the effects of renin, angiotensin and aldosterone (RAS) levels on the sinus rhythm restoration in patients with persistent atrial fibrillation. METHODS The sinus rhythm was restored in 36 patients with persistent atrial fibrillation using oral amiodarone and according to the result, all these patients were divided into two groups: restored group (n = 19) and unrestored group (n = 17 ). The plasma concentrations of RAS of the two groups were compared and their correlation to the left atrium diameter was also studied. RESULTS The concentrations of RAS in the restored group were significantly lower than those in the unrestored group (P 〈0.01, P 〈0.05). The concentrations of RAS were correlated significantly with the diameter of the left atrium ( P 〈 0.01 ). CONCLUSION It is relatively difficult to restore the sinus rhythm by orally taking amiodarone in atrial fibrillation patients with high levels of RAS.
