非钙化型肺结核瘤的动态CT增强研究

摘　　要：目的:探讨非钙化型肺结核瘤的动态CT增强特点。方法:对24例非钙化型结核瘤进行动态CT增强,观察其增强后的CT值、时间-密度曲线及强化特点。结果:24例非钙化型结核瘤中,20例增强后最大CT净增值均小于15 HU,平均(8.47±6.41)HU,时间-密度曲线为平坦型;4例结核瘤增强后最大CT净增值均明显大于15 HU,平均(60.16±17.26)HU,时间-密度曲线为上升型。所有结核瘤增强后无强化7例,边缘薄线型强化7例,轻度强化6例,明显强化4例。结论:(1)不活跃型结核瘤增强后最大净增CT值<15 HU,主要表现为无、轻度或边缘薄线型强化;(2)活跃型结核瘤增强后可见明显强化,需注意与恶性结节区分。Objective To evaluate the patterns of contrast-enhanced dynamic CT of noncalcified pulmonary tuberculomas. Method 24 patients with noncalcified pulmonary tuberculomas underwent dynamic enhanced CT. The increased CT value, time-attenuation curves and enhanced patterns were analyzed. Results Of 24 noncalcified pulmonary tuberculomas,the biggest increased CT value was less than 15 HU in 20 cases and the increased CT value was averagely （8.47 ± 6.41 ）HU ,whose time-attenuation curves were stable. The biggest increased CT value was more than 15 HU obviously in 4 cases and the mean increased CT value was （60.16 ± 17.26） HU,whose time-attenuation curves were in a ascending pattern. Of all tuberculomas,7 did not enhance,7 showed ring-like enhancement,6 showed mild enhancement,4 showed significant enhancement. Conclusions （ 1 ）The biggest increased CT value of inactive tuberculomas is less than 15 HU, no enhancement or mild or ring-like enhancement can be observed. （2）Significant enhancement can be seen in active tuberculomas,which must be distinguished from malignant pulmonary nodules.
