应用丝虫特异性IgG4检测试剂盒在湖南省消除丝虫病地区的监测

摘　　要：目的探讨丝虫特异性IgG4抗体ELISA检测试剂盒的敏感性和特异性,评价其在消除丝虫病地区监测的应用价值。方法应用山东省寄生虫病防治研究所和深圳市康百得生物科技有限公司联合研制的丝虫特异性IgG4抗体ELISA检测试剂盒,盲法检测滤纸干血样本共521份,并与病原学检查方法进行平行对照。结果检测原班氏微丝蚴血症转阴者76人,丝虫特异性IgG4抗体全部阴性,病原学血检亦未发现微丝蚴血症者;检测消除丝虫病地区居民69人,丝虫特异性IgG4抗体阳性2例,阳性率为2.90%,病原学血检未发现微丝蚴血症者。检测基本消除丝虫病后地区出生儿童143人和非流行区儿童200人,丝虫特异性IgG4抗体均为阴性;检测慢性丝虫病及其他寄生虫病病人33例,未检出丝虫特异性IgG4抗体阳性者;对血吸虫病、并殖吸虫病、华支睾吸虫病等未见交叉反应。结论丝虫特异性IgG4检测试剂盒敏感性和特异性较高,可以用于消除丝虫病地区监测的筛查和防治效果评价。Objective To test the validity of filaria-specific IgG4 ELISA kit, and evaluate the applicability of this kit in Hunan province. Methods 521 blood samples in dried filter papers were examined by using the filariaspecific IgG4 antibody testing kit jointly produced by Shandong Institute of Parasitic Disease and Shenzhen Combined Biotech Co. Ltd. The tests were performed in parallel with parasitological methods. Results Of the 76 cases with previous micrefilaremia, all of the samples were negative in the ELISA assay and microfilaremia was not found on the blood smears. Of the 69 adult inhabitants living in the filaria-free region, 2 samples showed the positive results by the ELISA method, and the positive rate was 2.90%. Microfilaremia was not found. In the study of 143 children living in endemic areas （but now free of filaria） and 200 children from the non-endemic regions, all samples were negative for filaria. In the study of 33 patients with chronic filariasis and patients with other parasitic diseases, positive result was also not observed. The kit is specific for filaria as cross-reaction with schistosoma, paragonimus, and clonorchiasis sinensis was not observed. Conclusion The filaria-specific IgG4 testing kit is sensitivity and specificity for filaria.
