曲普瑞林触发排卵的临床研究

摘　　要：目的:探讨促性腺激素释放激素激动剂(GnRH-a)曲普瑞林(triptorelin)触发排卵的疗效。方法:对应用克罗米芬和／或人绝经期促性腺激素(HMG)促排卵治疗排卵障碍患者,卵泡成熟时给予曲普瑞林(A组)或人绒毛膜促性腺激素(HCG)触发排卵(B组),卵巢过度刺激综合征(OHSS)高危周期均给予曲普瑞林,比较两组临床结果。结果:共分析了70例患者137个促排卵周期,两组周期数分别为71和66,两组排卵时间、周期排卵率、临床妊娠率、多胎率、OHSS发生率及流产率相似(P>0．05)。优势卵泡数达到4个时A组卵泡排卵率高于B组(P<0．05)。A组71个促排卵周期中有9个OHSS高危周期,发生3例中度OHSS。结论:曲普瑞林触发排卵方案卵泡排卵率高。Objective： To evaluate efficacy of gonadotrophin-releasing hormone analogue triptorelin in triggering ovulation. Methods ：70 patients with ovulatory dysfunction were administered with clomiphene or HMG. Once follicles matured, the patients were treated with either triptorelin（ A group）or hCG （B group）to trigger ovulation. For patients having a risk of ovarian hyperstimulation syndrome（OHSS） at their periods were treated with triptorelin only. Results ：137 stimulated ovulation cycles were analyzed, of which the number of cycles in A and B group was 71 and 66, respectively. No significant differences in respect of the ovulation time, ovulation rate per cycle, clinical pregnancy rate, multiple gestation rate, incidence rate of OHSS and abortion rate were found in both groups（P 〉 0.05）. Ovulation rate per follicle in the A group was higher than in the B group when the number of matured follicles was up to 4 （P 〈 0.05）. Of 9 patients having a risk of OHSS in A group, 3 developed OHSS. Conclusion： Triptorelin triggers ovulation rate per follicle higher.
