摘  要:目的探讨糖调节受损(IGR)患者血中游离脂肪酸(FFA)及各成分水平变化与动脉粥样硬化的关系。方法对120例糖尿病高危人群行OGTT,分为IGR、2型糖尿病(T2DM)和糖耐量正常(NGT)组,测定血中FFA及各成分的水平,超声测颈动脉内膜中层厚度(IMT)和动脉斑块的发病率。结果(1)与NGT组比较,IGR和T2DM组血清FFA和饱和脂肪酸(SFA)水平明显升高(P<0.05);(2)多不饱和脂肪酸(PUSFA)水平降低(P<0.01);IMT增厚,动脉斑块的发病率增高(P<0.05);(3)IMT厚度与血清SFA水平呈正相关,与PUSFA水平呈负相关。结论IGR患者血中FFA水平升高和PUSFA水平降低与颈动脉IMT增厚和动脉斑块的发病率相关。Objective To investigate the relationship between serum levels of free fatty acids and incidence of atherosclerosis (AS) in subjects with impaired glucose regulation(IGR). Methods The serum levels of free fatty acids were measured by using high-performance liquid chromatography in subiects with normal and impaired glucose regulation as well as type 2 diabetes. Arterial intimamedia thickness (IMT) and arterial plaque were measured by scanning carotid artery using ASPEN ultrasound with high resolution (L7, 7.5 MHz). Results (1) Compared with NGT group, levels of saturated fatty acids (SFAs) were higher (P〈 0. 05) and the levels of polyunsaturated fatty acids (PUSFAs) were lower (P〈0.05). (2) IMT and arterial plaque were markedly increased in IGR and T2DM versus NGT group. (3) Correlation analysis showed that SFAs levels were positively correlated with arterial IMT (CC value: 0. 2625, P〈0. 05). Serum PUFSAs levels were inversely correlated with IMT (r=-0. 2740, P〈0.05). Conclusions Elevated serum SFAs and decreased PUSFAs were characteristics of dyslipidemia in impaired glucose regulation. The change of FFAs might play a role in atherosclerosis formation of the patients with IGR and T2DM.
