糖尿病酮症酸中毒甲状腺激素水平观察及预后评估

摘　　要：目的:观察糖尿病酮症酸中毒(DKA)患者甲状腺激素(TH)水平的变化,探讨与其疾病的严重程度及预后的关系。方法:对我院2002年10月—2005年10月DKA住院病人观察组入院次日分别检测TT3、TT4、FT3、FT4、TSH、rT3,并取同期住院的非糖尿病、非甲状腺疾病的病人作对照组。将观察组与对照组比较,存活组与死亡组比较,存活组的治疗前后进行比较。结果:观察组的TT3、TT4显著低于对照组(P<0.05),rT3反而升高(P<0.05)。观察组中,死亡组TT3、TT4显著低于存活组(P<0.05),rT3反而高于存活组(P<0.05),存活组中治疗前TT3、TT4显著低于治疗后(P<0.05),rT3反而高于治疗后(P<0.05)。结论:DKA患者体内TT3、TT4水平下降、rT3升高,与疾病严重程度和预后有关。Objective： To observe the changes of level of thyroid hormones（ TH ）in patients with diahetic ketoacidosis （DKA）to explore the relationship between the severity and prognosis. Methods. The DKApatients were enrolled in this study （DKA group） and the non--DM and non--thyroid diseases patients serve as control group in our hospital from Oct 2002 to Oct 2005. level of TT3 ,TT4 ,FT3 ,FT4 ,TSH,rT3 was measured. The results were compared between the DKA group and the control group. Meanwhile, the comparison was done as well the survived and the dead as also before and after therapy in the DKA. Resuits： ① The level of TT3 ,TT4 level in DKA group were s but rT3 level was higher（P〈0.05） ;② In the DKA group gnificantly lower than that in the control group the level of TT3 and TT4 in the dead cases were significantly lower than that in the survived cases but rT3 level was higher（P〈0.05）;③In the survived group ,the level of TT3 ,TT4 were significantly higher after treatment than before treatment but rT3 level was lower（P〈0.05）. Conclusions： The decreased level of TT3 ,TT4 and increased rT3 in DKA patients associated with the severity and prognosis of DKA.
