经纤支镜介入微创治疗在中心气道器质性狭窄中的作用

摘　　要：目的:探讨经纤支镜介入微创治疗在中心气道器质性狭窄中的作用。方法:对286例确诊的中心气道器质性狭窄的患者,根据病因和病情轻重不同,在积极治疗原发病的同时,经纤支镜引导进行微波、电刀、激光、高压球囊扩张、支架置入、局部注射药物和灌洗综合治疗。结果:显效131例(45·8%)、有效126例(44·1%),总有效率为89·9%。pH值由术前的7·290±0·045上升至7·365±0·073(P<0·01),PaO2(mmHg)由术前61·46±0·09上升至82·38±0·24(P<0·01),PaCO2(mmHg)由术前的54·16±0·84下降至38·18±0·64(P<0·01),SaO2(%)由术前的88·4±2·1上升至94·5±3·4(P<0·01)。结论:对于中心气道器质性狭窄的患者及时进行纤支镜下介入微创治疗,有利于迅速解除气道梗阻,改善呼吸。纤支镜介入微创治疗技术是较为安全、有效、先进、简便,易被患者接受,值得临床推广应用。OBJECTIVE： To explore the efficacy of tiny-wound treatment by fibrobronchoscope catheter intervention in the organic constriction of center airway. METHODS： Two hundred and eithty-six patients suffered from the organic constriction of center airway were treated with fiberoptic bronchoscopy injection, contacting-microwave radiotherapy and dilatation of saccule combined local injection etc respectively according to their different situation besides coping with primary disease, actively. RESULTS： The improvement rate was 45,8% （131/286）, the effective rate was 44.9% （121/286）, and total effective rate was 89.90%. After the tiny-wound treatment by fibrobronchoscope catheter intervention, the blood gas analysis obviously improved, compared with those in the pre-treatment. The pH improved （7.290±0.045 vs 7.365±0.073 （P〈0.01）, the PaO2 heightened （61.46±0.09 vs 82.38±0.24） （P〈0.01）, the PaCO2 declined （54.16±0.84 vs 38.18±0.64） （P〈0.01）, and the SaO2 improved （88.4±2.1 vs 94.5±3.4） （P〈0.01）. CONCLUSIONS： For the patients with organic constriction of center airway, the tiny-wound treatment by fibrobronchoscope is a promising way to clear the airway obstruction, and it is an effective simple and safe method, with less complications and it could be widely used in clinical pratice.
