摘 要:目的探讨快速减压对家兔脊髓组织学和超微结构的影响。方法将18只实验兔随机分为对照组、减压病3 h组和减压病3 d组。5 min内用压缩空气加压至0.6 MPa,停留60 min,然后于3~5 min内减至常压。以上3组分别于进舱前、出舱后3 h和3 d,取脊髓进行光镜和透射电镜检查。结果(1)光镜显示脊髓灰白质散在出血灶,软脊膜及脊髓白质毛细血管内可见气泡;(2)电镜下显示脊髓白质毛细血管周围水肿,神经纤维变性、胶质细胞坏死。结论脊髓软脊膜及白质毛细血管内气泡,灰白质出血、水肿,神经纤维变性和胶质细胞坏死是减压病脊髓损伤发病的病理学基础。Objective To investigate the effects of quick decompression on spinal cord histology in rabbits. Methods 18 rabbits were put into the hyperbaric chamber. The pressure was increased to 0.6 MPa within 5 min using compressed air, maintained at 0.6 MPa for 60 min, and then was decompressed to normal pressure within 3~5 min. Spinal cord of the rabbits was removed and examined by light microscope and electromicroscope examination before entering the chamber, 3 h and 3 d after coming out of the chamber, respectively. Results (1) Through light microscope, petechial hemorrhages were observed in white and gray matter of spinal cord, and gas bubbles were found in the capillaries of the mater spinalis and spinal cord. (2) Through electromicroscope, perivascular tissue edema, nerve fiber degeneration and gliocyte necrosis in the white matter of the spinal cord were observed. Conclusions Gas bubbles in the capillaries in the mater spinalis and spinal cord, petechial hemorrhages in the white and gray matter of the spinal cord, perivascular tissue edema, nerve fiber degeneration and gliocyte necrosis may be the pathological mechanism of decompression sickness spinal cord injury.
