摘 要:目的探讨血清骨髓过氧化物酶(MPO)与急性冠状动脉综合征(ACS)的关系。方法对61例经冠状动脉造影(CAG)确诊的冠心病患者,根据病情分为:ACS组46例,包括急性心肌梗死(AMI)16例,不稳定型心绞痛(UAP)30例;稳定型心绞痛(SAP)组15例。另以24位健康人作为正常对照组。运用酶联免疫吸附法测定各组血清MPO浓度。结果ACS组、SAP组、正常对照组血清MPO质量浓度分别为(36·70±15·32)μg/L、(15·50±3·18)μg/L、(14·62±3·07)μg/L。此外,ACS组内AMI和UAP者血清MPO质量浓度分别为(46·89±13·63)μg/L和(31·27±10·09)μg/L。SAP组与AMI及UAP者之间MPO质量浓度差异具有统计学意义(P<0·01,P<0·01),SAP组与正常对照组之间差异无统计学意义(P>0·05)。血清MPO水平与白细胞之间呈明显正相关,但与C反应蛋白、总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、血小板计数、心肌肌钙蛋白I之间无明显相关性。结论ACS患者血清MPO质量浓度明显升高,与冠状动脉斑块的稳定性密切相关,提示血清MPO活性升高可能与ACS的发生有关,MPO是一种预测ACS的炎症标志物。Objective To investigate the relationship between the serum level of myeloperoxidase (MPO) and the degree of acute coronary syndrome (ACS). Methods According to clinical manifestation, coronary heart diseases (CHD) patients diagnosed by CAG were divided, including 16 patients with acute myocardial infarction (AMI), 30 patients with unstable angina pectoris (UAP), 15 with stable angina pectoris (SAP). AMI and UAP belonged to ACS group and 24 normal healthy served as control group. The serum level of MPO was detected by ELISA and was compared between the groups. Results MPO in the ACS group, SAP group, and control group was (36.70±15.32) μg/L, (15.5±03.18) μg/L, (14.62±3.07) μg/L, respectively. MPO in the AMI group and UAP group was (46.89±13.63) μg/L and (31.27± 10.09) μg/L, respectively. There were significant differences between AMI and SAP group (P 〈 0.01 ), UAP and SAP group ( P 〈 0.01 ), but there was no significant difference between SAP group and control group ( P〉0.05). There was positive correlation between the level of serum MPO and the number of WBC, but there was no obvious correlation between the level of serum MPO and the number of platelet and the level of CRP, TC, TG, HDL-C, LDL-C, TNI. Conclusion The concentration of MPO in ACS patients was increased and closely correlated with the stability of the plaque in coronary artery. This suggested that the increased level of MPO may be connected with the onset of ACS, and MPO could serve as an inflammatory marker for ACS.
