倍他乐克对肥厚型心肌病左室舒张功能的影响

摘　　要：目的观察静脉应用倍他乐克对肥厚型梗阻性心肌病(Hypertrophic Obstructive Card iomyopathy,HOCM)患者左室舒张功能、血流动力学和左室流出道压力阶差的影响。方法选取HOCM患者18例,采用多普勒超声诊断仪分别检测静脉应用倍他乐克前后左室舒张早期二尖瓣最大流速(E,m/s)、左室舒张晚期二尖瓣最大流速(A,m/s)、左室流出道压力阶差(LVOTPG,mm Hg)和每搏输出量(SV,m l)等反映左室舒张功能的指标。结果与静脉注射倍他乐克前相比,用药后E明显增高;A明显降低,E/A明显上升,LVOTPG明显下降;SV明显增加。结论静脉注射倍他乐克能显著改善HOCM左室舒张功能、降低左室流出道压力阶差以及增加每搏输出量。Objective To investigate the effect of betaloc on left ventricular diastolic function, kinetic parameter of blood stream and chang of left ventricular outflow tract pressure gradient in patients with hypertrophic obstructive cardiomyopathy（HOCM）. Methods 18 patients with hypertrophic obstructive cardiomyopathy were enrolled into the study. Maximal flow of mitral valve in earlier stage （E,m/s） and advanced stage of diastole（A,m/s）, left ventricular outflow tract pressure gradient （LVOTPG, mm Hg） and stroke volume （SV, ml） were measured. Results In comparison with left ventricular diastolic function before betaloc injection, E were significantly ascended, A were significantly descended, and E/A ratio were significantly ascended ; the pressure gradient of left ventricular outflow tract（LVOTPG） after betaloc injection were significantly descended ;and the stroke volume（SV） went up remarkably after betaloc injection. Conclusion Betaloc injection can significantly improve the diastolic function of left ventricle, ascend stroke volume, and descend left ventricular outflow tract pressure gradient in patients with HOCM.
