抗梅毒治疗对隐性梅毒患者细胞免疫的影响

摘　　要：目的研究人类免疫缺陷病毒(HIV)阴性隐性梅毒患者外周血T淋巴细胞亚群、自然杀伤(NK)淋巴细胞及白细胞介素-2受体(SIL-2R)的变化,并探讨其临床意义。方法应用流式细胞仪(FCM)及酶联免疫吸附法(ELISA)测定60例隐性梅毒患者(治疗组32例,未治疗组28例)和30例正常人外周血T淋巴细胞亚群及NK淋巴细胞及SIL-2R的水平。结果未治组隐性梅毒患者CD8淋巴细胞的比例(0.33±0.08)高于对照组(0.25±0.04)(P<0.05),CD4/CD8的比率(0.011±0.004)低于对照组(0.020±0.002),但治疗组和未治组间CD4/CD8的比率差异无统计学意义(P>0.05)。梅毒患者的SIL-2R水平[(450.16±90.33)KU/L]明显高于正常对照组[(269.80±62.75)KU/L],治疗后下降[(331.54±56.39)KU/L]。结论未治组和治疗组患者均存在细胞免疫抑制,这种异常可降低机体抵抗和消除梅毒螺旋体感染的能力。Objective To investigate the lymphocyte subsets,natural killer（NK） and soluble interleukin-2 receptors（SIL-2R） in syphilis patients with negative HIV and evaluate its clinical significance. Methods T-lymphocyte subsets,NK lymphocyte and SIL-2R were measured with flowcytometry（FCM） and enzyme linked immunosotbent assay（ELISA） from 60 cases of untreated group （28 cases）, treated group（ 32 cases） and controls group （30 cases） in patients with latent syphilis. Results The percentage of CD8 in untreated group （0.33 ± 0.08） was markedly higher than controls group（0.25 ± 0.04 ）（ P 〈 0.05 ）. The ratio of CD4/CD8 in untreated group（0.011 ± 0. 004 ） was notably lower than controls group（0. 020 ± 0. 002 % ） （ P 〈 0. 005 ）. If had no difference between treated group and controls group（P 〈 0.05 ）. The level of SIL-2R [ （450.16 ± 90.33 ）KU/L] was markedly higher than controls group[ （269.80 ± 62.75 ）KU/L]. The level of SIL-2R dropped[ （331.54 ± 56.39 ）KU/L] after treatment. Conclusion There is an imbalance for cellar immunity or immunity inhibition in untreated and treated in latent syphilis patients,which may weaken the ability of resistance and clearing treponema infection.
