摘 要:为提高左室肥大心电图诊断的敏感性和准确率,采用超声法对照研究340例16项常用传统心电图左室肥大标准的诊断价值,发现多数单项标准敏感性和准确性不甚理想.提出“新综合记分法”(∑QRS≥175mm、Rv_6>Rv_5、PTFV_1≤-0.04mm·s、QRS≥0.16s各记3分,ST-T改变、电轴左偏+29°——30°各记2分,总记分≥5判为左室肥大),诊断的敏感性和准确性可分别提高至66%(P<0.05)和81%(P<0.01).In order to improve the sensitivity and accuracy in electrocar-diographic diagnosis of left ventricular hypertrophy, 16 electro-cardiographic criteria for diagnosis of left ventricular hypertrophy in 340 cases were evaluated echocardiographically. It was found that most of voltage and non-voltage criteria, by method of degeneracy and series versus parallel connection, be merged into a new comprehensive score system, which combines major ones (including ∑QRS≥175mm, Rv6>Rv5 ,PTFVl≤-0.04mm ?s, QRS≥0. 16s, any of them scored 3 points) with minor ones (including left axis deviation + 29?-30? ST-T changes, eitherof them scored 2 points). The total score≥5 was seen as a criterion for left ventricular hypertrophy. The new score system was demonstrated to be of high sensitivity (66%) and significant accuracy (81%). Thus, electrocardiogram would remain one of most simple and useful methods for diagnosis of left ventricular hypertrophy on bedside.
