Caspase-8及Bcl-2表达与胎膜早破的相关性研究

摘　　要：目的：探讨Caspase-8及Bcl-2表达与胎膜早破的关系。方法：选择2003年6月～2004年5月我院足月妊娠自然分娩的孕妇48例，其中24例发生胎膜早破（胎膜早破组），24例未发生胎膜早破（对照组），病例均于阴道分娩后取胎膜破裂口处的胎膜组织5cm×5cm大，同时胎膜早破组在距胎膜破口处10cm以上的部位再取同样大的胎膜组织，胎膜组织均经石蜡包埋切片后采用免疫组化法测定Caspase-8及Bcl-2的表达。结果：（1）两组病例的胎膜组织均可见Caspase-8及Bcl-2的表达；（2）在胎膜早破组的胎膜组织中Caspase-8表达的阳性单位为6．89±0．19，对照组为2．33±0．06（P〈0．01）；而Bcl-2表达的阳性单位为9．55±0．24，对照组为21．37±0．32（P〈0．01）；（3）在胎膜早破组非破口部位胎膜组织中Caspase-8及Bcl-2表达的阳性单位分别为6．93±0．17和9．66±0．19，与破口部住胎膜组织中Caspase-8及Bcl-2表达的阳性单位比较无差异（P〉0．05）。结论：胎膜早破的发生与Caspase-8的过度表达及Bcl-2表达的下调相关。Objectlve：To investigate the expressions of Caspase-8 and Bcl-2 in human fetal membrane associated with premature rupture of membrane （ PROM ）. Methods： Fetal membranes from 48 full-term normal vaginal deliveries were included in the study. Twenty-four of 48 deliveries had PROM. Twenty-four deliveries served as controls. In the control group, fetal membranes were taken from the rupture site. In PROM group, membranes were taken from the rupture site, and 10 cm far away from the rupture side. Expression of Caspase-8 and Bcl-2 in the membranes of both groups was studied by immunohistochemistry and image analysis. Results： （1） Expression of Caspase-8 and Bcl-2 could be observed in the fetal membranes of both groups; （2） In PROM and control groups, positive units of Caspase-8 expression were 6.89 ± 0. 19 and 2.33 ±0.06 ,positive units of Bcl-2 expression were 9.55 ±0.24 and 21.37 ±0.32. There were significant differences in expression of Caspase-8 and Bcl-2 between PROM and control group（P 〈0.01） ;（3） Within PROM group,expressions of Caspase-8 and Bcl-2 had no significant difference in the rupture site and 10cm far away from the rupture site. （P 〉0. 05 ）. Conclusion：Excessive expression of Caspase-8 and decrease in Bcl-2 expression are related to the occurrence of PROM.
