代谢综合征患者心率变异性与心室重塑的关系

摘　　要：目的检测代谢综合征(MS)患者心率变异性(HRV)改变,探讨其与心室重塑的关系。方法采用holter检测系统检测了47例MS患者HRV时域指标SDNN、SDANN、SDNNI、RMSSD和PNN50及超声心动图指标LVd、LVs、LVPWI、Vs、LVEF、E/A、AO,并以30例健康体检者作为对照。结果与对照组比较,NMS患者SDNNI、RMSSD、PNN50降低,而在HMS组中SDNN、SDANNS、DNNI、RMSSD、PNN5O均降低,且RMSSD、PNN5O在NMS和HMS组间有统计学差异;MS患者无论是否存在高血压,其LVd、LVs、LVPW均较正常升高而E/A降低,合并高血压时上述指标变化更明显,且LVs增厚和AO增宽;多因素相关分析显示RMSSD、PNN50与E/A、LVd相关,SDNNI与LVdI、VS相关。结论MS患者HRV下降,并与心脏结构和功能的改变有关。Objective To explore the changes of heart fete variability（HRV） and its relationship with echocardicgraphyic parameters in patients with metabolic syndrcme. Methods HRV parameters including SDNNN, SDANN, SDNNI, RMSSD and PNN50 and echocardicgraphyic parameters including LVd, LVs, LVPW, IVS, LVEF and E/A were measured in 47 patients with metabolic syndrcme and 30 healthy persons acted as control group. Results HRV parameters including SDNNI, RMSSD and PNN50 in patients with metabolic syndrcme and normal blood pressure decreased when compared with that of controls, while all HRV parameters in patients with metabolic syndrome and high blood pressure decreased. Statistic differences of RMSSD and PNN50 were also found between metabolic syndrome patients with normal and high blood pressure. Multivariable analysis（stepwise regression） showed that E/A and LVd were independendy associated with RMSSD and PNN50, while LVd and IVS were associated with SDNNI. Conclusion The disequilibrium of autonomic nerve system and the decrease of HRV exist in patients with metabolic syndrome and relate to ventricular remolding.
