呼吸机及人工气道所引起呼吸附加功的临床研究

摘　　要：目的气管插管等器械导致的附加功可能使机械通气患者呼吸功增高，评价附加功对脱机有指导意义。方法通过ＢｉｃｏｒｅＣＰ－１００呼吸监测仪监测术后患者在不同水平压力支持通气（ＰＳ）、Ｔ管及拔管后的呼吸功等参数变化，并推算克服附加功所需的ＰＳ水平。结果随ＰＳ水平降低患者呼吸功显著增加。器械导致的附加功使ＰＳ０ｋＰａ时的呼吸功增加５７％，而气管插管使Ｔ管时的呼吸功增加４１％。克服附加功的ＰＳ水平为０．６６ｋＰａ。结论当ＰＳ水平过低或采用Ｔ管时。objectiveTheimposedworkofbrethingcausedbythebrethingapparatus(endotra-chealtube,breathingcircuittubing,andventilator′svalves)mayincreasepatient′sworkofbreathing.Toevalu-atetheimposedworkmaybeusefultodetermineweaning.MethodsWorkofbreathingwasobservedbyBicoreCP-100breathingmonitoratdifferentlevelspressuresupport(PS),Tpieceandaf-terextubation,andthePSleveltocompensateimposedworkofbreathingwascomputed.ResultsWhenPSlevelde-creased,patient′sworkofbreathingincreasedsignificantly.AtPS0kPa,thepatient′sworkofbreathingin-creased57%,whichwascausedbyimposedworkofapparatus.Becauseoftheresistanceofendotrachealtube,thepatient′sworkofbreathingincreased41%atTpiece.ThePSlevelthatcompensatedfortheim-posedworkwas0.7kPa.ConclusionsWhenPSlevelwastooloworTpiecewasapplied,theimposedworkofbreathingcausedbyapparatuscouldmarkedlyincreasepa-tient′sworkofbreathing.
