摘 要:目的通过研究高压电烧伤病人足甲襞微循环变化,探讨高压电烧伤对人体微循环的影响。方法用WX-9B及WX-753B型微循环显微镜及微循环图像处理系统,观测50例高压电烧伤病人足甲襞微循环,并同50例健康人对照。高压电烧伤组病人均有电流出入口,体表烧伤面积为(12.4±9.3)%,平均年龄(26.58±5.33)岁;正常对照组平均年龄(24.40±4.68)岁。观察指标包括微血管形态、微血流动态、微血管周围状态共三方面十六项指标。结果高压电烧伤组与正常对照组比较,前者微血管清晰度降低,管襻数减少,输入枝及输出枝长度缩短,输入枝、输出枝和襻顶直径变小,血流速度减慢,红细胞聚集性增高,襻周有渗出和出血。结论高压电烧伤对人体外周微循环有明显影响。Objective To explore how high voltage electrical injury affects human body microcirculation by studying the changes of foot nailfold microcirculation(FNM) in patients with high voltage electrical injury. Methods Observing FNM of 50 patients with high voltage electrical bums and other 50 healthy adults as controls by using WX-9B, WX-753B microcirculatory microscopy with image analysis system. The patients in electrical burns group have the entrance and outlet of current in common, with a burn area of 12.4 ± 9.3%, age averaging 26.58±5.33. Age of patients in control group averaging 24.40 ± 4.68. The observation indexes include three aspects with 16 indexes as microvascular morphology, flow state and peripheral state. Results Compared with the control group, in electrical burn group, the distinctness of microvascular and the number of vessel loop decreased, length of efferent and afferent branches becomes shorter, diameters of efferent and afferent branches and the top of the loops become lessen, velocity of blood flow is slower, erythrocyte aggregation becomes heavier, peripheral exudation and hemorrhage can be seen. Conclusion High voltage electrical injury has a great effect on human body periphery microcireulation.
