强直性脊柱炎骶髂关节病变CR及CT诊断的对比分析

摘　　要：为探讨强直性脊柱炎(AS)骶髂关节改变的CR及CT诊断特点,评估病变程度,以提高其诊断及鉴别诊断能力。回顾性分析76例AS患者的骶髂关节CR及CT表现特点并进行临床分析。结果显示AS骶髂关节病变的CR正位及斜位片表现特点为骶髂关节间隙变窄,关节面致密硬化、囊变及关节强直;CT表现特点为病变主要累及骶髂关节滑膜部髂骨侧,软骨钙化、关节间隙变窄、关节面及面下骨质结构囊变、硬化以及骶髂韧带钙化。表明CR正位及斜位片可以显示骶髂关节的全貌,并能排除其他病变,对Ⅲ、Ⅳ级的AS可以明确诊断,但对≤Ⅱ级的AS难以显示;CT具有较高的分辨率,对AS的骶髂关节病变具有更高的敏感性,能够显示CR片难以显示的早期病变,便于测量,能够对AS准确分级,是诊断AS的主要检查方法。In order to explore the characteristics of the CR and CT diagnoses of sacro-iliac joint changes in ankylosing spondylitis(AS),the CR and CT image characteristics of the sacro-iliac joints of 76 cases of AS were retrospectively analyzed together with the clinical manifestations as to evaluate the lesion severity and improve the correct rates of the diagnosis and differential one.The results showed that the anteroposterior and oblique CR films of the sacro-iliac pathological change in AS were characterized by: sacro-iliac space stenosis,pyknosis,sclerosis and cystic change of the articular surface and joint ankylosis; CT ilms were characterized by: iliac sacro-iliac synovium being the main lesion-involved site,cartilage calcification,joint space stenosis,cystic change and calcification of the articular surface and the bone structure under the surface and sacro-iliac ligament calcification,suggesting that the anteroposterior and oblique CR films can display the whole appearance of the sacro-iliac joint,exclude other diseases and offer the confirmed diagnosis for AS of types III and IV but fails to display AS ≤ type II;CT has a high resolving power and a high sensitivity to the sacro-iliac joint diseases in AS and can display the early pathological changes that CR fails to display,being the main method for the diagnosis of AS due to its convenience for the measurement and accurate gradation of AS.
