聚丙烯酰胺水凝胶注射隆乳失败后826例硅胶囊假体再隆乳临床报告

摘　　要：目的:探讨注射聚丙烯酰胺水凝胶(PAHG)隆乳失败后的并发症和清除PAHG的方法及硅胶囊假体(SBI)同步再隆乳的体会。方法:在907例注射PAHG失败后采用乳晕和腋窝切口在内窥镜直视下分层次较彻底清除PAHG后,对适合SBI隆乳的826例患者同步给予再次隆乳。结果:注射PAHG隆乳并发症较多,部分注入乳腺腺体内的PAHG和乳腺组织形成果冻样硬块及在胸肌组织中扩散,造成炎症性纤维增生反应和淋巴细胞及异物巨细胞浸润。对907例中826例行SBI再隆乳,术后效果良好。结论:作为注射隆乳失败的补救,SBI在清除注射PAHG后同步再隆乳是安全可行可靠的。Objective To inquire into the complication of failed augmentation mammoplasty by injecting PAHG （Polyacryamide Hydrogel）, and look for an effective way to ablate the PAHG and share the experiences of SBI （Silicone breast implants） synchronic breast reconstruction. Methods 907 patients joined in this study. Through an axillary and areola approach, the PAHG implants that were distributed in different levels were separated and ablated entirely under the endoscope. After the removal of the PAHG, only 81 patients were not fit for SBI synchronic breast reconstruction and the rest were given it. Results More postoperative complications were imputed to PAHG injection, which may cause damage to some lymph cells and lead to inflammatory proliferation of fiber tissues. Conclusion As a remedy to failed PAHG injecting augmentation mammaplasty, SBI implanting is much more reliable, safe and actualized.
