摘 要:目的探讨定量组织多普勒速度成像(QTVI)技术评价正常小儿心肌同步性运动的规律。方法获取标准心尖四腔观及两腔观,应用QTVI技术描记100例正常小儿左心室各节段及右心室侧壁基底段心肌的组织多普勒速度曲线,分别测量各节段心肌运动曲线上自QRS波起始至心肌收缩期峰值速度和舒张早期峰值速度的时限Ts和Te。结果(1)正常小儿左心室4个壁同一壁内3个节段Ts测值无显著性差异(P>0.05);而后间隔及前壁与侧壁及下壁之间Ts测值差异有显著性(P<0.05),余左室各壁间Ts测值差别无显著性(P>0.05);(2)右室侧壁基底段Ts测值较左室4个壁基底段Ts测值均值延迟(41.0±15.7)ms(P<0.01);(3)左心室壁各节段之间Te测值无明显差别(P>0.05)。结论正常小儿同一室壁内不同节段心肌的舒缩运动是同步的,但个别室壁之间在生理情况下同步性收缩存在有一定的差异,这可能是小儿的一个特点。Objective To evaluate the inter- and intraventricular synchrony in normal children. Methods Quantitative tissue velocity imaging (QTVI) was performed in 100 normal children on apical four-chamber view and two- chamber view. The time from onset of QRS complexes to peak velocity during systole and diastole (Ts and Te) was measured in myocardial movement curves of every segments. Results ①The Ts had no difference in different segments of the same wall in LV (P〉0. 05); There was a difference of Ts in the septum, anterior wall, lateral wall and inferior wall. ②The Ts of the basal segment of lateral wall in RV was about 41.0± 15.7 msec later than that of LV (P〈0.01) . ③The difference of Te in LV different segments had no statistical significance (P〉 0. 05 ) . Conclusions The systolic and diastolic movement of different segments in the same wall was highly synchronized and there was slightly difference of systolic synchrony in some LV walls. These may be the characteristics of normal children.
