摘  要:目的探讨M ark2(第2代)抗环瓜氨酸肽(CCP)抗体酶联免疫吸附试验(ELISA)检测类风湿性关节炎(RA)的临床应用价值。方法用M ark2和M ark1抗CCP抗体ELISA试剂盒对65例RA患者、154例其他非RA患者同时进行M ark2和M ark1抗CCP抗体检测;用速率散射比浊法检测患者血清类风湿因子(RF)浓度。结果M ark2抗CCP抗体对RA诊断的敏感性为73.8%,特异性为96.8%,诊断效率为90.0%,约登指数为0.71;而RF对RA诊断的敏感性为87.7%,特异性为81.8%,诊断效率为83.6%,约登指数为0.69;两者联合检测敏感性为96.9%,特异性为81.2%,诊断效率为84.7%,约登指数0.78。M ark1抗CCP抗体对RA诊断的阳性率为61.5%,不如第2代抗CCP抗体。结论M ark2抗CCP抗体可做为RA的辅助诊断指标。Objective To explore the diagnostic value of Mark2 anti-cyclic citrullinated peptide(anti-CCP) in patients with rheumatoid arthritis (RA). Methods In 65 RA patients, 154 not RA patients, serum rheumatoid factor (RF) was determined by nephelometry, Markl and Mark2 anti-CCP were tested by enzyme-linked immunosorbent assay (ELISA). Results The diagnostic sensitivity of Mark2 anti-CCP and RF in RA patients was 73.8% and 87.7%. After combined determination of anti-CCP and RF the positive rate rose to 96.9%. The diagnostic specificity of Mark2 anti-CCP and RF was 96.8% and 81.8% in RA. The diagnosis efficiency was 90.0% for Mark2 anti-CCP and 83.6% for RF. The youden's index was 0. 71 for Mark2 anti-CCP and 0. 69 for RF. The diagnostic specificity of combined determi- nation of Mark2 anti-CCP and RF was 81.2% , the diagnosis offieiency was 8d. 7% and the youden's index was 0. 78. The positive rate of Markl anti-CCP in RA was 61.5%, which were worse than Mark2. Conclusions Mark2 Anti-CCP could be regarded as a diagnostic marker in RA.
