彩超对肾实质强回声病变良恶性鉴别诊断的价值

摘  要:目的探讨彩色多普勒超声在肾实质内小的局灶性强回声病变良恶性鉴别诊断中的价值。方法对经CT、MR或手术病理证实的31例肾实质内小的局灶性强回声病变的二维、彩色多普勒图像及血流频谱进行回顾性分析。结果8例病变内部及周边可见彩色血流信号,同时频谱显示为动脉及静脉;20例病变内及周边无明显血流信号,频谱无显示。3例病变内及周边均可探及血流信号,频谱显示为静脉。结论8例病变内及周边探及动、静脉血流信号为恶性病变(均经手术病理证实为肾透明细胞癌)。23例无血流信号及有静脉血流信号的为良性病变(其中20例为错构瘤,3例为血管瘤)。本文结果显示,彩色多普勒超声对肾实质内小的局灶性强回声病变良恶性鉴别诊断有一定的应用价值,可无创伤的为手术前组织病理学判断提供参考信息。Objective To study the value of Color Doppler Ultrasound used in diagnosing pathological changes between malignant and benign of small focal strong echo in solid renal. Methods Retrospectanalysis of 2D - CDUS about 31 small focal strong echo in solid renal pathological changes which have been confirmed by CT, MR or operation. Results Color blood stream signal can be seen inside and around 8 pathological changes, and frequency spectrum shows artery and vein;No obvious color blood stream signal can beseen inside and around 20 pathological changes, and frequencyspectrum shows nothing; Inside and around 3 pathological changes we can find blood stream, and frequency spectrum shows vein. Conclusion 8 instances inside and around which artery and vein blood stream signal can be seen are malignant pathological changes. 23 instances inside and around which no blood stream signal or vein blood stream signal can be seen are benign pathological changes. The result indicates that Color Doppler Ultrasound has applicable value in diagnosing pathological changes between malignant and benign of small focal strong echo in solid renal, and it can provide helpful information for histopathology judgement before operation without hurt.
