摘 要:目的应用16层螺旋CT 3D(VR,M IP)重建技术,分析研究肾动脉的解剖变异及其规律。方法随机选择144例因为腹部病变而行16层螺旋CT增强的患者作回顾性肾动脉CTA分析。在AW 4.1工作站采用层厚1.25mm作3D(VR,M IP)图象重建。结果肾动脉变异分别占病例数和肾脏数的51.4%(74/144)和33.7%(97/288)。肾门前肾动脉分支(Ⅰ类)为最常见型,占变异数的56.7%(55/97),肾副动脉(Ⅱ类)次之,占35.1%(34/97),Ⅲ类(Ⅰ类和Ⅱ类混合存在型)少见,占7.2%(7/97)。肾动脉变异率在双肾及男女之间无差异。结论肾动脉变异颇为常见,表现形式多样,充分了解这些变异对腹腔镜肾脏切除术至关重要。Objective : To evaluate the visualization rate aud describe the morphologic pattern of the arteriorenal anatomical variants on muhidetector CT (MDCT) by using 3D reformation(VR,MIP) display. Methods: We conducted a retrospective evaluation of the arteriorenal anatomical variants with enhanced MDCT in 144 consecutive patients with 288 kidneys. The images were reeonstrncted with sections of 1.25 - mm thickness reconstructed . Results: Arteriorenal anatomical variants were noted in 51.4% of patients and 33.7% kidneys. Of all the arteriorenal anatomical variants, category Ⅰ ( proximal arterial branches ) were found in 56.7%, category Ⅱ ( accessory renal arteries ) 35.1% and category Ⅲ ( the mixture of category Ⅱ and category Ⅱ) 7.2%. In anatomical variant rate, there is no difference between the right and left kidneys, and between the male and female. Conclusion: Arteriorenal anatomical variants constitute the most common, clinically important renal vascular variant. It is extremely important for.surgeons who operate laparoscopic nephrectomy to have a full knowledge of these anatomical variants.
