摘　　要：目的研究肺癌患者外周血T淋巴细胞亚群的水平及变化特点。方法采用流式细胞技术,对41例肺癌患者术前检测外周血T淋巴细胞亚群水平,并与50例健康献血者作对照研究。结果与对照组比较,肺癌患者CD3+、CD4+、CD8+和NK细胞减少,CD4+/CD8+降低,差异均有统计学意义(P均<0.01)。淋巴细胞亚群变化与分期相关:Ⅰ期肺癌患者T淋巴细胞亚群未见明显抑制;Ⅱ期CD3+、CD4+、CD8+和NK细胞减少,但较Ⅰ期差异无统计学意义;Ⅲ期肺癌患者所有亚群均降低,分别与Ⅰ期和Ⅱ期比较,差异均有统计学意义;随着分期增加,CD4+/CD8+明显降低,各期之间差异均有统计学意义。淋巴结转移组NK细胞和CD4+/CD8+均明显低于无淋巴结转移组,差异均有统计学意义(P均<0.01)。结论肺癌患者存在免疫功能低下,免疫功能随着肿瘤的进展进一步受到削弱。Objective To investigate the alteration of T lymphocyte subsets in patients with lung cancer. Methods T lymphocyte subsets and nature killer cell （ NK cell） were detected in peripheral blood by flow cytometry. Blood samples were obtained preoperatively from 41 patients with lung cancer. Blood samples were also obtained from 50 healthy blood donors as controls. Results CD3^＋ , CD4^＋ , CD8^＋ , CD^4＋/CD8^＋ value and NK cell in lung cancer group were significantly lower than those in healthy controls （P 〈0.01 ）. No significant depression of T lymphocyte subsets was observed in lung cancer of stage i. CD3^＋ , CD4^＋ , CD8^＋ and NK cell were decreased in stage Ii, but there was no significant difference with that of stage Ⅰ （ P 〉 0. 05 ）. All T lymphocyte subsets decreased evidently in stage Ⅲ, and there were significant difference with that in stage I and Ⅱ respectively. With the increasing of TNM staging, the CD4^＋/CD8^＋ value decreased dramatically. The changes were significant difference among the stages. The CD4^＋/CD8^＋ value and NK cell showed significant decrease in patients with lymph nodes metastasis compared with those without lymph node metastasis（P 〈0. 01 ）. Conclusion The host immunity is impaired in patients with lung cancer. With the progression of lung cancer, the immunological function is further suppressed.
