数字X线技术诊断■原发性牙根纵裂的体外研究

摘　　要：目的：评价直接数字化影像技术在下颌第一磨牙牙根纵裂诊断中的作用。方法：选择牙根无缺失外形基本完整有牙根纵裂和无牙根纵裂的离体下颌第一磨牙各9颗，用胶泥将牙根包埋后随机编号，采用平行投照法以不同的水平投照角度（-5°，0°，5°，10°，15°）拍摄数字化X线片，4个观察者独立诊断通过数字化x线片能否发现牙根纵裂。统计结果，采用卡方检验比较结果的差异。结果：各角度的观察结果中0°、5°和-5°准确率、特异度、敏感度比10°和15°的结果好，但是结果在统计学上没有差异（P〉0．05）。不同观察者得到的结果有较好的一致性（k=0．57～0．65）。结论：数字x线片对下颌第一磨牙原发性牙根纵裂的诊断有良好的敏感度、特异度和准确率。Objective ：To evaluate the contribution of radiogram in diagnosis of primary vertical root facture in mandibular first molar. Methods：Eighteen extracted mandibular first molars were radiographed in mesio-distal planes at the project angle of -5° ,0° ,5°, 10° and 15° respectively. Four dentists examined all the films individually to judge if there was a primary vertical root fracture in each tooth. The accuracy, specifity and sencitivity of the fracture diagnosis were assessed, Kappa value among the examiners was calculated. Results：The radiographs at the various project angles did not result in satistic difference in accuracy, specifity and sencitivity of the fracture diagnosis（ P 〉 0.05 ）. The assessments of the 4 examiners were uniform（ K = 0.57 - 0.65 ）. Conclusion： Radiograph can provide essential evidence for the clinical diagnosis of the vertical root fracture in mandibular first molar.
