不同疾病血瘀证量化诊断的比较研究

摘　　要：目的:从病证结合的角度探讨与血瘀证相关的症状体征在不同疾病中表现的差异。方法:进行多中心、大样本临床流行病学血瘀证量化诊断调查,运用Logistic回归分析考察各个症状体征在冠心病、高血压病、脑梗塞三种疾病中贡献度大小。结果:3种病血瘀证诊断的危险因素分别为:冠心病:口唇色黑、肌肤甲错、眼周色黑、舌质紫黯、舌体瘀斑、舌脉曲张、五心烦热;高血压:腭黏膜征、齿龈色黑、头痛、恶心呕吐、舌质紫黯、舌脉曲张;脑梗塞:职业、头痛、腭黏膜征、肌肤甲错、颈项强痛、偏瘫、舌质紫黯、舌体瘀斑、舌脉曲张、脉涩。结论:相同证型在不同疾病中表现不完全相同,症状体征对于血瘀证诊断贡献度也不相等。Objective： To explore the expressional differentiation of symptoms and signs associated with blood stasis syndrome in different diseases. Method ： Putting up multi - center, large - sample epidemical survey about blood stasis syndrome, and using Logistic regression analysis to seeing about the contribution of each symptom and sign in coronary heart disease, hypertention disease, and cerebral infarction. Result ： The diagnosis risk factors of blood stasis syndrome in coronary heart disease are black oral lips, coarse skins, black rim of eye, dark purple tongue, petchia in tongue body, curve vessel in tongue, burning sensation of five centres. The diagnosis risk factors of blood stasis syndrome in hypertention disease are mucous membrand sign in palate, black gingiva, headache, nausea and vomit,dark purple tongue,curve vessel in tongue. The diagnosis risk factors of blood stasis syndrome in cerebral infarction are vocupation, headache, mucous membrand sign in palate, coarse skins, stiffness and pain in the neck, hemiparalysis, dark purple tongue, black oral lips, petechia in tongue body, curve vessel in tongue, and unsmooth pulse. Conclusion ： The expressions of same syndrome are not homology in different diseases, and the contribution of symptoms and signs are not equal for the diagnosis of blood stasis syndrome.
